摘  要:目的 :探讨电针改善椎动脉型颈椎病 (VCS)所致感觉神经性听力损失的机理。方法 :采用组织硬化剂注射法建立家兔VCS慢性椎 基底动脉供血不足 (VBI)模型。从注射后第 6周开始 ,电针组家兔行双侧“风池”“听宫”“外关”穴电针治疗 2周 ,分别记录 1 0Hz低刺激率、50Hz高刺激率听性脑干反应 (ABR)和低频 (1 0 0 0Hz)、高频 (6 0 0 0Hz)短纯音耳蜗电图 (EcochG)听神经动作电位(AP)阈值 ,将 3组结果进行比较。结果 :模型组 50Hz高刺激率ABR的Ⅲ波峰潜伏期和Ⅰ~Ⅲ波峰间潜伏期较对照组显著延长 (P <0 0 5 ,P <0 0 1 ) ,电针组与对照组差异无显著性意义 (P >0 0 5)。各组之间 1 0Hz低刺激率ABR差异无显著性意义 (P >0 0 5)。模型组和电针组AP阈值显著高于对照组 (P <0 0 1 ) ,模型组 6 0 0 0HzAP阈值显著高于 1 0 0 0HzAP阈值 (P <0 0 5) ,电针组 1 0 0 0Hz和 6 0 0 0HzAP阈值均显著低于模型组 (P <0 0 5 ,P <0 0 1 )。结论 :电针可改善VBI的脑干神经元突触效能和外周听觉通路传导 ,增强耳蜗高、低频听力水平 。ObjectiveTo explore the mechanism of electroacupun cture improving the cervicogenic sensorineural hearing loss induced by vertebroarterial type cervical spondylosis (VCS). MethodsChronic vertebro basilar insufficiency (VBI) rabbit model of the VCS was established by injection of tissue sclerosing remedy-775 into the solt tissue. From the 6th weeks after the injection, electroacupuncture treatment was given at bilateral Fengchi (GB 20), Tinggong (SI 19) and Waiguan (TE 5) for 2 weeks in the electroacupuncture group. The auditory brainstem response (ABR) evoked by click at low (10Hz) and high (50Hz) stimulating rates, and the threshold of the cochlear nerve act ion potential (AP) in electrococheogram (EcochG) induced by low (1 000Hz) and h igh (6 000Hz) frequency tone were recorded respectively from the left side of ear s. The results were compared. ResultsThe Ⅲ wave peak latency (PL) and the Ⅰ-Ⅲ wave interpeak latency (IPL) of ABR at 50Hz high stimulatin g rate in the model group became longer than those in the control group (P<0 05,P<0 01), but with no significant difference between the electroacupuncture group and the control group (P>0 05). There was no significant difference in ABR at 10Hz low stimulating rate among the three groups (P>0 05). The thresholds of AP were higher in the model group and the electroacupuncture group than those in the control group (P<0 01); and those at 6 000Hz frequ ency were significantly higher than those at 1 000Hz in the model group (P<0 05); and at 1 000Hz and 6 000Hz in the electoacupuncture group were si gnificantly lower than those in the model group (P<0 05,P<0 01). ConclusionElectroacupuncture can improve the neuron synapses efficiency of the brainstem and the conduction of the peripheral cochlear path, and raise the cochlear sensory levels for high and low frequency sound, and recovers the function of hearing in VBI.
