白虎汤加减在肾移植术后感染高热中的应用

摘　　要：目的 :分析肾移植术后感染高热的成因、诊断和治疗 ,探讨中西医结合方法治疗术后高热的机制。方法 :32例肾移植术后患者随机分为两组。治疗组采用中药白虎汤加减 ,激素、抗病毒、抗细菌等联合用药 ,全身支持疗法 ,减少环孢素 A用量 ,停用细胞毒药物。对照组未用中药 ,其余用药同治疗组。结果 :治疗组 15例中痊愈 14例 ,好转 1例 ,治愈率为 93% ;对照组 17例中痊愈 9例 ,好转 1例 ,死亡 7例 ,治愈率为 5 3% ;统计学处理 ,两组治愈率有显著差异 (P<0 .0 5 )。结论 :肾移植术后患者表现为邪充斥三焦 ,邪热虽盛 ,津液未伤 ;白虎汤有清热除烦、生津止渴的功效 ,对大热、大渴、脉数有力者有效 ,同时有降温、抗炎等作用 ,配合西药抗菌及全身支持疗法 。Objective: To analyze the reason, diagnose and treatment of high fever caused by infection after renal transplantation, and investigate the mechanism of integrated traditional Chinese and western medicine therapy in treating high fever after renal transplantation. Methods: Thirtytwo patients after renal transplantation were divided into two groups. The treatment group was treated with traditional Chinese medicine Baihu decoction(白虎汤), hormone, antivirus and antigerm treatment. At the same time the dosage of cyclosporin(CsA) was reduced, cellular toxic medicine was stopped using and whole body support treatment was used. The control group wasn't treated with traditional Chinese medicine. The other therapy was the same in the two groups. Results: Of 15 cases in the treatment group, 14 cases recovered, 1 case improved, the cured rate was 93%. Of 17 cases in the control group, 9 cases recovered, 1 case improved, 7 cases died, the cured rate was 53%. The cured rate of the two groups had significant difference(P<0 05). Conclusion: The patient after renal transplantation appears evil fire fulled the three cavities, though evil heat is strong, body fluids are not damaged. Baihu decoction has the effect of getting rid of fever and agitation, producing fluids and preventing thirsty, reducing temperature and antiinflammation etc. It has the effect in the patients with high fever, extreme thirsty, pulse quickly. After renal transplantation, the infective fever caused by low immune can be effectively controlled with Baihu decoction, antigerm western medicine and whole body support therapy.
