社区与医院双向转诊制度的建立和服务的开展

摘　　要：目的　为理顺医院与社区之间的关系 ,降低医疗费用、方便患者就近就诊、提高社会效益和经济效益。方法　建立了医院和社区之间一系列快捷的转诊制度和流程 ,成立双向转诊办公室专职机构和人员 ,实行每日出入院患者资料的收集和转诊管理 ,落实治疗、保健、康复服务 ,健康教育服务 ,加强双向转诊跟踪服务。结果　提高了社区居民对社区医疗服务的信任 ,巩固了患者的治疗效果 ,降低了患者的治疗费用 ,缩短了治疗时间 ,增加了医院的社会效益和经济效益。Objective To make clear of the relationship between hospitals and community,reduce the cost of healthcare,provide convenience for the patients to get healthcare nearby,and increase the social and economic benefits.Method We established a series of system and work flow for trans-hospital between hospitals and community,which including setting up office and allocating full-time personnel which deal with the affaires of trans-hospital,collecting the information of the patients admitted or discharged everyday,fulfilling the service of treatment,healthcare, recovery and health education,strengthening the service of follow-up after trans-hospital.Result This practice improved the community residents’ confidence in community health service, strengthened the effectiveness of treatment, reduced the cost and time of treatment, increased the social and economic benefits of the hospital.Conclusion More solid work need to be done to perfect the system of bi-directional trans-hospital.
