温阳化瘀法治疗系统性硬皮病的临床观察

摘　　要：【目的】探讨温阳化瘀法对系统性硬皮病 (SSc)患者的临床疗效及对外周血循环内皮细胞、皮质醇水平的影响。【方法】采用温阳化瘀法 ,以阳和汤加味治疗系统性硬皮病 16例 ,观察系统性硬皮病的临床症状、体征 ,并检测血浆循环内皮细胞、皮质醇水平的变化。【结果】经两个疗程 (6个月 )的治疗后 ,16例SSc患者中临床痊愈 1例 ,显效 4例 ,有效 8例 ,无效 3例 ,总有效率为 81 2 5 %。实验指标检测结果显示 ,患者的血管内皮细胞水平降低 ,皮质醇水平升高 ,与治疗前比较差异均有显著性 (P <0 0 1)。【结论】温阳化瘀法是治疗系统性硬皮病的有效方法 ,有改善患者临床症状和体征的作用 ,其作用机理可能与保护血管内皮细胞和调节内分泌—免疫网络系统作用有关。To investigate the effect of yang-warming and blood-stasis-removing therapy for systemic scleroderma (SS) and to observe its influence on plasma levels of vascular endothelial cells (VEC) and cortisol.Sixteen cases of SS were treated with modified Yanghe Decoction and the symptoms and signs, plasma levels of VEC and cortisol were observed before and after treatment. After 6-month treatment,one case was cured,4 markedly effective,8 effective and 3 ineffective,the total effective rate being 81.25%.As for the result of laboratory examination,VEC level was decreased and cortisol level increased, the difference being significant as compared with those before treatment (P<0.01).[Conclusion]Yang-warming and blood-stasis-removing therapy is effective for SS in improving symptoms and signs and its mechanism may be related to the protection of VEC and the regulation of endocrine-immune function.
