昆山市社区卫生服务需要与利用分析

摘　　要：采用两阶段抽样方法对昆山市张浦、花桥、陆家3镇997户居民进行了入户调查,以了解该市居民社区卫生服务需要与利用水平和经济负担情况。结果显示昆山市居民的整体健康状况良好,医疗卫生服务需求和利用较低,但其需求满足状态较好。病轻和自我医疗是未就诊的主要原因,医院的服务质量影响居民住院。居民就医流向较合理,符合“大病进医院,小病进社区”的模式。同时,地段医院经济负担较轻。由于妇幼保偿制、计划免疫和妇女保健服务较好。建议加强健康宣传教育和用药指导,进一步完善社区卫生服务补偿机制和价格管理,使社区卫生服务健康发展。This article is based on the investigation among 997 households in three townships by two stage sampling method in order to find out the need and utilization of Community Health Services in Kunshan City,Jiangsu province.The results show that the health status of pupulation in Kunshan city is good,the level of their need and medical services is lower than that of other cities,the utilization is not high,but it is satisfied in demand.The main causes of not using the outpatient services are light illness and self treatment.The quality of hospital services impact the utilization of being in bospital.The flow of patients among the hospitals is in resaon and adapt to the Model of 'severe diseases treated in hospital,light one treated in Community'.At the same time,the economic burden of patients in Community hospital is lower than that of the city and district hospitals.With the coverage of maternal and child health insurance there is a high rate of immunization and fine woman health care.It is suggest to strengthen the health education,guide practice of medicine,consummate the reimbursement mechanism and management of price for developing the Community Health Services.
