摘 要:目的 通过对普通外科常见手术病种的住院流程进行重组 ,缩短平均住院日 ,增加住院患者人数 ,降低平均住院费用 ,增加医院医疗业务收入。方法 将北京大学第三医院普通外科病房住院手术患者分为干预手术病种与非干预手术病种两组 ,对 2 0 0 0年 1~ 9月与 2 0 0 1年 1~ 9月两组患者的平均住院日、出院人数、医疗业务收入、平均住院费用进行对比评价及统计分析。结果 两组患者的出院人数均有增长 ,平均住院日均有缩短 ,医院医疗业务收入有所增长 ,差异有统计学意义(P <0 .0 5 )。平均住院费用虽有所降低 ,但差异无统计学意义 (P >0 .0 5 ) ,且平均住院费用较 2 0 0 0年增高。结论 作业流程重组对缩短平均住院日、增加住院患者人数、增加医院医疗业务收入有效。单纯缩短平均住院日并不能完全降低患者住院费用。Objective To investigate the business process reengineering of common kinds of operations in the department of general surgery, so as to shorten the average days of hospitalization, reduce the average hospitalization cost, and increase the income of medical treatment in hospital.Methods The department of general surgery in the Third Hospital of Peking University was chosen.The patients were divided into intervention group and controlled group.The average days of hospitalization, the numbers of discharged patients,the income of medical treatment,and the average hospitalization cost from January to September in 2000 and 2001 were compared between the two groups.Results In both groups,the numbers of discharged patients and the income of medical treatment increased, the average days of hospitalization was shortened significantly (P<0.05). The average hospitalization cost had no change (P>0.05). However, the average hospitalization cost increased compared with that in 2000.Conclusions Business process reengineering was effective to shorten the average days of hospitalization and increase the income of medical treatment. Shortening the average days of hospitalization singly can not decrease the average hospitalization cost.
