摘 要:目的 :探讨抗G 蛋白偶联型受体血管紧张素Ⅱ 1型受体 (AT1受体 )和肾上腺素能α1受体 (α1受体 )自身抗体是否与糖尿病心肌病发病有关。方法 :以合成的AT1与α1受体多肽片段为抗原 ,应用酶联免疫吸附测定技术 ,检测 196例住院及门诊体检者血清中抗G 蛋白偶联型受体AT1和α1受体自身抗体 ,其中糖尿病心肌病患者 46例 (糖尿病心肌病组 ) ,2型糖尿病 5 2例 (糖尿病组 ) ,高血压无靶器官受损患者 5 8例 (高血压组 )及正常对照组 40例。结果 :糖尿病心肌病组抗AT1受体自体抗体阳性率和抗α1受体自身抗体阳性率 ,明显高于糖尿病组、高血压组以及正常对照组 ,有显著性差异 (P均 <0 .0 5~ 0 0 1)。结论 :免疫学机制可能参与糖尿病心肌病病理生理过程。Objective: To explore the role of autoantibodies against AT_1-adrenergic receptor(AT_1-receptor)and α_1- receptor in the development of diabetic cardiomyopathy. Methods: The epitopes of the second extracellular loop of AT_1 receptor (165-191) and α_1receptor (192-218) were synthesized and used respectively to screen serum autoantibodies from patients with diabetic cardiomyopathy (DC) ( n =46),diabetes mellitus (DM)( n =52),or hypertension ( n =58) and from healthy blood donors ( n =40,control) by ELISA. Results: In diabetic cardiomyopathy,the positive rates of autoantibodies against AT_1 -receptor and α_1-receptor were 50% and 36.8%,respectively. The positive rates were both higher than those of patients either with DM (13.4% and 9.6%,respectively) or with hypertension(15.5% and 12.1%,respectively ). They were also higher than those of healthy donors (10% and 12.5 %,respectively; p <0.01). Conclusion: The findings showed higher positive rates of autoantibodies against AT_1-receptor and α_1 -receptor in patients with diabetic myocardial disease. These autoantibodies may play important roles in the pathogenesis of diabetic cardiomyopathy.
