摘　　要：目的　观察带血管蒂大转子骨瓣治疗股骨头缺血性坏死的疗效。方法　根据解剖学研究 ,临床应用以旋股内侧动脉深支和臀下动脉为蒂的大转子骨瓣治疗股骨头缺血性坏死 16例 2 0侧。结果　全部病例经 2 6年随访 ,髋关节功能按Harris髋关节指数评价 ,优 9例 ,良 7例。结论　该手术增加股骨头的血运 ,疗效确定 ,适合青壮年股骨头缺血性坏死各期病变的治疗。Objective To investigate the efficacy of the vascular-pedicle flap of the greater trochanter for the treatment of avascular necrosis of the femoral head. Methods The greater trochanter bone flap with the deep branch of the medial femoral circumflex artery and the inferior gluteal artery was used to treat avascular necrosis of the femoral head in 20 thighs of 16 patients on the basis of anatomic study. Results All patients were followed up for 2～6 years. The function of the hip was assessed by Harris,s score. Nine cases were rated as excellent and 7 fine. Conclusion This method can augment the blood supply of the femoral head and fits for the treatment of every stage of avascular necrosis of the femoral head in young and middle-aged patients.
