瘦素及胰岛素样生长因子-Ⅰ与胎儿生长受限的关系

摘　　要：目的　探讨瘦素及胰岛素样生长因子Ⅰ (IGF Ⅰ )与胎儿生长受限 (FGR)的关系。方法　采用放射免疫法和免疫放射法测定 30例FGR孕妇 (FGR组 )和 80例正常孕妇 (对照组 )的血清及脐血瘦素、IGF Ⅰ水平 ,并对其结果进行相关性分析。结果　FGR组血清瘦素水平与对照组比较 ,差异无显著性 (P >0 0 5 ) ;FGR组血清IGF Ⅰ水平明显低于对照组 (P <0 0 5 ) ,FGR组脐血瘦素、IGF Ⅰ水平均明显低于对照组 (P <0 0 5 ,P <0 0 1 ) ;两组孕妇血清瘦素、IGF Ⅰ水平与脐血瘦素、IGF Ⅰ水平无相关性 (r =0 1 85 ,r =0 2 6 2 ,P >0 0 5 ) ;脐血瘦素水平与新生儿出生体重呈正相关 (r =0 36 4 ,P <0 0 5 ) ,与胎盘重量无相关性 (r =0 1 94 ,P >0 0 5 ) ;脐血IGF Ⅰ水平与新生儿出生体重及胎盘重量呈正相关 (r =0 4 75 ,r =0 4 86 ,P <0 0 5 )。结论　FGR孕妇脐血瘦素水平降低与胎儿脂肪沉积减少有关 ,脐血瘦素水平可作为预测胎儿体重的一项指标。血清与脐血IGF Ⅰ的分泌相对独立 ,提示IGF Ⅰ不能通过胎盘屏障。脐血IGF Ⅰ水平降低 ,可能是导致FGR的病因之一。Objective To study the relationship between fetal growth restriction(FGR) and levels of leptin and insulin like growth factor Ⅰ(IGF Ⅰ).Methods Immunoradioassay was used to measure the leptin and IGF Ⅰ levels of serum and cord blood in 30 pregnant women with FGR(FGR group) and 80 normal pregnant women (normal group).Results The difference of serum leptin levels between the FGR group and the normal group was of no significance(P>0 05).The serum IGF Ⅰ level in the FGR group was obviously lower than that in the normal group(P<0 05).Cord blood leptin and IGF Ⅰ level in FGR group were notably lower than those in normal group (P<0 05,P<0 01).Serum leptin and IGF Ⅰ were not correlated with those in cord blood in the two goups (r=0 185,r=0 262,P>0 05);Cord blood leptin level was positively correlated with fetus birth weight (r=0 364,P<0 05) and was not correlated with placenta weight (r=0 194,P>0 05);cord blood IGF Ⅰ level was positively correlated with fetus birth weight and placenta weight(r=0 475,r=0 486,P<0 05).Conclusion The decreased cord blood leptin level in FGR is correlated with failed fetal fat deposition,which can be an index in detecting fetal birth weight.The separation of IGF Ⅰ in maternal serum from that in cord blood indicates that IGF Ⅰ cann't go through the placental barrier.Decrease of cord blood IGF Ⅰ maybe another cause of FGR.
