摘  要:目的:探讨微创颅内血肿清除术在高血压脑出血治疗中的优越性及其对神经功能早期康复的影响。方法:将78例高血压脑出血患者分为A、B两组,A组:微创组42例,在内科治疗基础上行CT片定位法微创颅内血肿清除术;B组:开颅组36例,在内科治疗基础上开骨瓣或骨窗行血肿清除术。比较两组患者治疗后的有效率、死亡率和治疗前后神经功能缺损评分。结果:开颅组有效率为55.6%,死亡率为11.1%;微创组有效率90.5%,死亡率为4.8%。两组比较有显著性差异(P<0.05)。比较两组术后神经功能缺损评分也有显著性差异(P<0.05)。结论:微创颅内血肿清除术在高血压脑出血治疗中能提高有效率、降低死亡率,结合早期康复治疗能促进神经功能恢复。Objective:To evaluate the advantages of mini-invasive evacuation of hematoma for the treatment of hypertensive intracerebral hemorrhage(HICH) and to observe the effect of the recovery of neurological function.Method:78 cases with HICH were grouped into A and B:Forty-two cases in group A received mini-invasive surgery medical treatment,36 cases in group B received craniotomy and medical treatment.The effective rate,mortality and neurological deficit scale of the two groups were compared.Result:The effective rate and mortality rate in group A were 90.5% and 4.8%, which was significantly different from group B(55.6% and11.1%,P<0.05).Conclusion:The mini-invasive evacuation of hematoma can improve the effective rate,decrease the mortality rate.
