隐神经营养血管蒂逆行岛状皮瓣修复小腿远端骨外露的疗效

摘　　要：目的探讨隐神经营养血管蒂逆行岛状皮瓣修复下肢软组织缺损的临床效果。方法对9例小腿下1/3踝关节周围皮肤坏死及缺损的患者,行隐神经营养血管蒂逆行岛状皮瓣转移修复术,皮瓣面积最大12cm×8cm,最小6cm×5cm,平均9cm×6.5cm。结果8例全部成活,1例部分坏死,随访6～12个月,皮瓣质地满意,外观佳。结论隐神经营养血管蒂逆行岛状皮瓣修复下肢软组织缺损,取材方便,血供可靠,不牺牲主要动脉。Objective To study the clinical effects of repair of soft tissue defects at the lower limbs by transplanting the retrograde island skin flap pedicled with saphenous nerve and nutrient vessel. Methods 9 cases of cutaneous necrosis and defect around the ankle and 1/3 lower part of the leg were managed with transfer of the reversed saphenous neurocutaneous island flaps. The sizes of the flap ranged from 12×8cm to 6cm×5cm,with an average of 9cm×6.5cm. Results 8 flaps survived except one which had partial necrosis. They were followed up from 6 months to 12 months. The flaps were fine both in quality and appearance. Conclusion It is a convenient and reliable method to repair the soft tissue defects with the above mentioned flap, for it can offer good blood supply without sacrifice of major arteries.
