Cardiovascular and cerebrovascular diseases, including heart disease and stroke, pose significant challenges for patient rehabilitation and management. With technological and research advancements, the LACE model (considering Length of stay, Acuity of admission, Comorbidities, and Emergency department visits) has increasingly become a key tool in optimizing post-discharge patient management. This article focuses on the current status of patients with cardiovascular and cerebrovascular diseases, discussing the role and clinical value of the LACE model in predicting post-discharge risks and improving patient management. Through a comprehensive analysis of existing literature, it aims to provide references for future research, especially on how to refine the application of the LACE model and its combination with other strategies to promote the long-term health and well-being of patients with cardiovascular and cerebrovascular diseases.
