“医院–社区–家庭”一体化护理照顾模式在肾移植术后患者的应用效果研究

摘　　要：目的:探讨“医院–社区–家庭”一体化护理照顾模式在肾移植术后患者的应用效果。方法:选取2017年5月~2017年10月在本院进行肾移植术后患者38例作为对照组,2017年11月~2018年3月在本院进行肾移植术后患者46例作为干预组,对照组在院内给予常规手术后护理干预,干预组在对照组基础上采用“医院–社区–家庭”一体化护理照顾模式进行干预,比较两组患者术后3个月的生理功能、生理职能、躯体疼痛、生活功能、心理健康指标和营养状况。结果:干预组的生理功能、生理职能、躯体疼痛、生活功能、心理健康指标均显著高于对照组,有统计学意义(P【0.05);术后三个月,干预组的营养状况显著优于对照组,有统计学意义(P 【0.05)。结论:“医院–社区–家庭”一体化护理照顾模式可显著改善肾移植术后患者生活质量,对于降低并发症的发生,改善营养情况有重要临床价值。Objective: To explorethe application effect of “hospital-community-family” integrated nursing caremodel in patients after kidney transplantation. Methods: 38 patients afterrenal transplantation in our hospital from May 2017 to October 2017 wereselected as the control group, and 46 patientsafter renal transplantation in our hospital from November 2017 to March 2018were selected as the intervention group. The control group was given routinepost-operative nursing intervention in the hospital, and the intervention groupwas intervened in the “hospital-community-family” integrated nursingcare model on the basis of the control group.
