老年2型糖尿病和糖耐量受损患者血清铁蛋白水平的观察

摘　　要：目的观察老年2型糖尿病(T2DM)和糖耐量受损(IGT)患者血清铁蛋白水平的变化,以明确老年糖尿病人群铁代谢的特点。方法入选T2DM 316例,IGT 133例,正常组228例;基线检测了体质指数(BMI)、空腹血糖(FBG)、餐后2 h血糖(2 hBG)、空腹胰岛素水平以及血清铁蛋白水平(SF),采用稳态模式(HOMA)评价胰岛素抵抗(HOMA-IR),3年后进行了相同的随访,并比较随访前后各组间血清SF水平的差异。结果基线时和3年后,IGT组和T2DM组血清SF均高于对照组(P<0.05,P<0.01)。但T2DM组3年后血清SF水平较基线时有所下降(P<0.01)。基线时SF与空腹胰岛素和2 hBG独立相关,3年后SF与FBG、空腹胰岛素和胰岛素抵抗指数独立相关。结论基线时和3年后老年IGT组和T2DM组血清SF均高于正常组,SF与胰岛素抵抗正相关,老年T2DM人群3年后SF水平较基线时有所下降。Objective To explore the correlation between serum ferritin and insulin resistance in elderly patients with type 2 diabetes mellitus(T2DM) and impaired glucose tolerance(IGT).Methods 316 cases of T2 DM,133 cases of IGT group,288 cases of control group were enrolled in this study.Body mass index(BMI),fasting blood glucose,2 h blood glucose and serum ferritin were measured.Insulin resistance was evaluated by the homeostasis model.Results The level of serum ferritin and HOMA-IR in both IGT and T2 DM groups were higher than controlgroup at the baseline and 3years later GP< 0.05,P<0.01).And serum ferritin level in T2 DM group was decreased after 3 years.At the baseline,the serum ferritin was independently correlated with fasting insulin and 2 h blood glucose(P< 0.05).HOMA-IR was also independently correlated with serum ferritin after 3 years CP<0.05).Conclusions Serum ferritin in T2 DM and IGT were both higher than control group in elderly patients.And in elderly T2 DM patients,serum ferritin level was decreased after3 years.
