实时三维超声心动图评价房间隔缺损患者介入封堵前后右室形态及收缩功能

摘　　要：目的探讨实时三维超声心动图(RT-3DE)在评价不同程度肺动脉收缩压(PASP)的房间隔缺损(ASD)患者介入封堵术治疗前后右心室形态及收缩功能变化中的应用价值。方法选择经超声心动图确诊为单纯继发孔型房间隔缺损患者35例按PASP分为PASP正常组(Ⅰ组)、PASP轻度增高组(Ⅱ组)、PASP中度增高组(Ⅲ组),选取20例健康体检者作为对照组,分别测量各组ASD患者介入封堵术前1周、术后3 d及对照组右室舒张末期容积(RVEDV)、收缩末期容积(RVESV)、每搏量(RVSV)、射血分数(RVEF)并进行统计分析。结果Ⅱ组、Ⅲ组患者术后PASP水平较术前减低(P均<0.05),Ⅰ组术后PASP水平与术前差异无统计学意义(P>0.05);三组患者术前RVEDV、RVESV、RVSV水平均较对照组升高(P均<0.05),术后RVEDV、RVSV水平较术前减小(P均<0.05);Ⅰ组、Ⅱ组术后RVEF水平较术前减低(P均<0.05),Ⅲ组术后RVEF与术前差异无统计学意义(P>0.05)。结论 RT-3DE可以用于评价不同程度PASP的ASD患者介入封堵术后右心室形态及收缩功能变化。Objective To evaluate the value of assessment of right ventricular morphology and function by real-time three dimensional echocaediography echocardiography in artial septal defect patients with different pulmonary artery systolic pressure before and after interventional percutaneous closure(IPC).Methods 35 atrial septal defect(ASD)patients in general hosptial of ningxia medical university were divided into the three group according to the degree of pulmonary artery systolic pressure(groupⅠ-normal group,groupⅡ-mild group,groupⅢ-moderate group).20 healthy controls were involved as the control group.Right ventricular end diastolic volume(RVEDV),right ventricular end systolic volume(RVESV)and right ventricular ejection fraction(RVEF)were measured by RT-3DE in all patients before and after IPC and in healthy controls.Results After IPC the PASP of groupⅡand Ⅲ group lower than before(P<0.05),the PASP of group Ⅰwas no significant difference compared with before IPC(P>0.05).Before IPC,RVEDV,RVESV,RVSV of three groups were higher than control group.After IPC the RVEDV,RVSV of the three groups were significantly lower than before(P<0.05).After IPC the RVEF of groupⅠand groupⅡ significantly lower than before(P<0.05),while the RVEF of group Ⅲ was no significant difference compared with before IPC(P>0.05).Conclusion RT-3DE Echocardiography could be used for assessment of right ventricular norphology and function by real-time 3D echocardiography in artial septal defect patients with different pulmonary artery systolic pressure before and after IPC.
