胸腔镜食管癌根治术联合胸内吻合术治疗食管癌的效果观察

摘　　要：目的探索胸腔镜食管癌根治术联合胸内吻合术治疗食管癌的效果。方法选取2016年10月-2019年7月收治的80例食管癌患者,随机分成2组,对照组40例采取开胸食管癌根治术联合胸内吻合术,观察组40例实施胸腔镜食管癌根治术联合胸内吻合术。比较两组的手术基本指标以及不良事件发生情况;并对比两组的治疗前后肺功能改善情况。结果观察组手术操作总时间和平均出血量、手术不良事件发生率均低于对照组(P<0.05)。而两组在淋巴结清扫总数的对比上相当(P>0.05)。治疗前二组患者肺功能情况比较,无统计学意义(P>0.05),而治疗后观察组肺功能情况的改善更优异(P>0.05)。结论食管癌患者实施胸腔镜食管癌根治术联合胸内吻合术效果确切,可减少手术的时间和减少术中的出血量,减少术后手术不良事件的发生,更好改善肺功能,且根治效果和开胸手术相似。Objective To explore effect of thoracoscopic radical esophagectomy combined with intrathoracic anastomosis in treatment of esophageal cancer.Methods The paper chose 80 patients with esophageal cancer treated from October 2016 to July 2019,and divided them into two groups randomly.40 cases in control group were treated with radical esophagectomy combined with intrathoracic anastomosis,and 40 cases in observation group with thoracoscopic radical esophagectomy combined with intrathoracic anastomosis.Basic indexes of operation and adverse events incidence was compared between two groups.Pulmonary function improvement before and after treatment was compared between two groups.Results Total operation time,average bleeding volume and adverse events incidence in observation group was lower than control group,(P<0.05).Comparison of total number of lymph node dissections between two groups was similar,(P>0.05).There was no significant difference in pulmonary function between two groups before treatment(P>0.05),that in observation group was better than after treatment,(P>0.05).Conclusion Thoracoscopic radical esophagectomy combined with intrathoracic anastomosis can achieve accurate effect for patients with esophageal cancer,can reduce operation time and bleeding amount,postoperative adverse events incidence,and improve lung function,with similar radical effect to thoracotomy.
