睾丸原发性非霍奇金淋巴瘤20例临床病理分析

摘　　要：目的探讨睾丸原发性淋巴瘤(PTL)的临床病理学特征、鉴别诊断、治疗及预后等。方法回顾性分析20例PTL患者的临床病理资料,总结其临床病理特点、免疫表型等,并复习相关文献。结果20例PTL病例中,患者年龄44~80岁,中位年龄65.5岁,均表现为睾丸不同程度的肿大。20例PTL中17例(85.0%)为弥漫大B细胞淋巴瘤(DLBCL),且均为非生发中心起源(non-GCB)型。2例(10.0%)为套细胞淋巴瘤(MCL),其中1例为CD5阴性的套细胞淋巴瘤,1例为母细胞变异型套细胞淋巴瘤。1例(5.0%)为外周T细胞淋巴瘤,非特殊型(PTCL,NOS)。结论PTL是一种罕见的结外淋巴瘤,老年人多见,临床主要表现为睾丸无痛性肿大,PTL进展快、预后差,结合组织病理学特点及免疫组织化学检查可作出明确诊断。Objective To investigate the clinicopathological features,differential diagnosis,treatment and prognosis of primary testicular lymphoma(PTL).Methods The clinicopathological data of 20 patients with PTL were retrospectively analyzed,their clinicopathological features and immunophenotype were summarized,with review of the relevant literature.Results Among the 20 cases of PTL,age of the patients was 44-80 years with median 65.5 years.17 out of 20 PTL cases(85.0%)were diffuse large B cell lymphoma(DLBCL),and all the 17 cases were non-germinal center origin(non-GCB)type.2 cases(10.0%)were mantle cell lymphoma(MCL).Among them,1 case was CD5 negative mantle cell lymphoma and 1 case was mantle cell lymphoma,blastic variant(BV-MCL).1 case(5.0%)was T-cell lymphoma,non-specific type(PTCL-NOS).Conclusion PTL is a rare type of extranodal lymphoma.It is easily found in the elderly people.The main clinical manifestations are painless swelling of the testis.PTL progresses quickly and the prognosis is poor.Combined with pathological features and immunohistochemical examination,a definite diagnosis can be made.
