PTEN蛋白表达与中国人群非小细胞肺癌风险相关性的Meta分析

摘　　要：目的使用Meta分析系统评价第10染色体同源缺失性磷酸酶-张力蛋白酶(tensin homologue deleted on chromosome 10,PTEN)蛋白表达与中国人群肺癌发生的相关性。方法计算机检索Pubmed、Embase、中国期刊全文数据库、维普中文期刊数据及万方数据库。纳入有关PTEN蛋白表达与中国人群非小细胞肺癌发生风险及不同临床病理特征相关性的病例-对照研究。结果共纳入27篇文献,所纳入文献具有齐性,应用固定效应模型;不齐,应用随机效应模型,结果显示非小细胞肺癌组PTEN蛋白表达量明显低于非肿瘤肺组织,差异有统计学意义(OR=0.14,95%CI 0.11~0.17,P<0.01),在鳞癌和腺癌中PTEN蛋白表达量比较,差异无统计学意义(OR=1.05,95%CI 0.87~1.28,P>0.05),低分化肺癌组PTEN蛋白表达量明显低于中高分化组,差异有统计学意义(OR=0.30,95%CI 0.24~0.37,P<0.01),C+D分期的肺癌组PTEN蛋白表达量明显低于A+B期,差异有统计学意义(OR=0.32,95%CI 0.25~0.41,P<0.01),肺癌转移的患者PTEN蛋白表达量明显低于未转移组,差异有统计学意义(OR=0.42,95%CI 0.25~0.69,P<0.01)。结论低PTEN表达与临床分期晚、肿瘤组织分化程度低、早期侵袭转移的可能性成高相关,且PTEN蛋白表达情况对肺癌早期诊断和预后判断都有一定的参考价值。Objective To evaluate the correlation between the expression of homologous deleted phosphatase on chromosome 10(PTEN)protein and the occurrence of lung cancer in a Chinese population based on a meta-analysis.Methods Pubmed,Embase,the full text database of Chinese journals,the data of Weipu Chinese journals,and the Wanfang database were searched by computer.A case-control study on the correlation between PTEN protein expression and non-small cell lung cancer risk and different clinicopathologic characteristics in a Chinese population was conducted.Results A total of 27 homogeneous articles were included.Fixed effect,uneven,and random effect models were used.The results showed that the expression of PTEN protein in the non-small cell lung cancer group was significantly lower than the non-tumor lung tissue group(OR=0.14,95%CI 0.11-0.17;P<0.01).There was no significant difference in the expression of PTEN protein between squamous cell carcinoma and adenocarcinoma(OR=1.05,95%CI 0.87-1.28;P>0.05).The expression of PTEN protein in the low differentiation lung cancer group was significantly lower than the middle and high differentiation lung cancer groups(OR=0.30,95%CI 0.24-0.37;P<0.01).The expression of PTEN protein in the lung cancer group(C+D stages)was significantly lower than stages A+B(OR=0.32,95%CI 0.25-0.41;P<0.01)and the expression of PTEN protein in patients with lung cancer metastases was significantly lower than patients without lung cancer metastases(OR=0.42,95%CI 0.25-0.69;P<0.01).Conclusion Low expression of PTEN is highly related to the late clinical stage,low differentiation of tumor tissue,and the possibility of early invasion and metastasis.The expression of PTEN protein has a reference value for the early diagnosis and prognosis of lung cancer.
