口腔扁平苔藓患者血清、唾液中的MMP-13表达意义分析

摘　　要：目的探究口腔扁平苔藓(Oral Lichen Planus,OLP)患者血清及唾液中基质金属蛋白酶-13(MMP-13)的表达意义。方法选取新疆某医院口腔门诊2018年1月-2021年1月接受治疗的60例OLP患者为研究组,选择同期于新疆某院接受体格检查的50例健康个体为对照组,对比两组个体血清及唾液MMP-13、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平,评估研究组患者血清与唾液MMP-13因子水平同OLP疾病活动积分(REU)的相关性,最后评估血清、唾液MMP-13对OLP的诊断价值。结果(1)比较显示,研究组患者的唾液与血清中MMP-13、TNF-α与IL-6水平均明显高于对照组,组间差异明显(P<0.05);(2)开展Spearman相关性分析可以发现,研究组患者的血清MMP-13与唾液MMP-13表达水平均与患者REU得分呈正相关(r=0.9581、r=0.9749,P<0.05);(3)开展ROC曲线分析可以发现,唾液MMP-13对OLP诊断AUC分别为0.9931(95%CI为0.9749-1.000)、0.9758(95%CI为0.9334-1.000)(P<0.05)。结论OLP患者的血清、唾液中会出现炎症因子异常表达的现象,且MMP-13同OLP患者疾病严重程度呈正相关性,可以考虑将MMP-13作为OLP诊断指标之一推广于临床应用中。Objective To explore the expression significance of matrix metalloproteinase-13(MMP-13)in the serum and saliva of patients with Oral Lichen Planus(OLP);Method Choose to receive treatment in our hospital from January 2018 to January 202160cases of OLP patients in the study group were selected as the study group,and 50 healthy individuals who received physical examinations in our hospital during the same period were selected as the control group.The serum and saliva MMP-13,tumor necrosis factor-α(TNF-α)and leukocyte-mediated To evaluate the correlation between serum and saliva MMP-13 factor levels and OLP disease activity score(REU)of patients in the study group,and finally evaluate the diagnostic value of serum and saliva MMP-13 for OLP;Results(1)The comparison showed that the saliva and serum levels of MMP-13,TNF-α,and IL-6 in the study group were significantly higher than those in the control group,with significant differences between groups(P<0.05);(2)Spearman correlation The analysis showed that the expression levels of serum MMP-13 and saliva MMP-13 in the study group were positively correlated with the patient’s REU score(r=0.9581,r=0.9749,P<0.05);(3)ROC curve analysis showed that,The diagnostic AUC of saliva MMP-13 for OLP was 0.9931(95%CI0.9749-1.000)and 0.9758(95%CI 0.9334-1.000)(P<0.05);Conclusion Inflammatory factors appear in the serum and saliva of OLP patients The phenomenon of abnormal expression,and MMP-13 is positively correlated with the severity of the disease in OLP patients,it can be considered to promote MMP-13 as one of the diagnostic indicators of OLP in clinical applications.
