HIF1-α和PCDGF在鼻咽癌诊断和预后评价中的临床价值研究

摘　　要：目的:探讨鼻咽癌患者缺氧诱导因子-1α(HIF-1α)及畸胎瘤细胞源性生长因子(PCDGF)表达情况及与临床、预后关系。方法:选择我院2014年2月-2015年2月间诊治的鼻咽癌患者62例为观察组,健康志愿者40例为对照组,比较两组HIF-1α及PCDGF表达情况。观察鼻咽癌患者HIF-1α及PCDGF表达相关性,分析HIF-1α及PCDGF表达与预后关系。结果:观察组HIF-1α阳性表达率83.9%(52/62),PCDGF阳性表达率90.3%(56/62),均显著高于对照组的5.0%(2/40)和7.5%(3/40),差异有统计学意义(x^2=60.709,68.396,P<0.05)。观察组中,T3及T4期、Ⅲ及Ⅳ期、有淋巴结转移者HIF-1α高表达、PCDGF高表达、HIF-1α和PCDGF高表达的比例明显高于T1及T2期,Ⅰ及Ⅱ期,无淋巴结转移者,差异有统计学意义(P<0.05)。Spearman相关分析显示鼻咽癌患者HIF-1α与PCDGF表达呈正相关(r=0.856,P<0.01)。随访36周,患者整体生存率为65.7%。HIF-1α和PCDGF低表达、HIF-1α高表达、PCDGF高表达、HIF-1α和PCDGF高表达鼻咽癌患者生存率分别为87.5%、56.7%、67.9%和52.2%,四组生存情况比较,差异有统计学意义(x^2=4.433,P<0.05)。结论:鼻咽癌患者HIF1-α和PCDGF具有高表达,其表达水平与患者临床、预后间关系密切。Objective:To investigate the expression of HIF-1αand PCDGF in patients with nasopharyngeal carcinoma and its relationship with clinical and prognosis.Methods:62 patients with nasopharyngeal carcinoma who were diagnosed and treated in our hospital from February 2014 to February-2015 were selected as the observation group,and 40 healthy volunteers as the control group.The expressions of HIF-1αand PCDGF in two groups were compared.expression of HIF-1αand PCDGF in patients with nasopharyngeal carcinoma were observed,the relationship between the expression of HIF-1αalpha and PCDGF and the prognosis were analyzed.Results:The positive expression rate of HIF-1αin the observation group was 83.9%(52/62),and the positive expression rate of PCDGF was 90.3%(56/62),which was significantly higher than that in the control group 5%(2/40)and 7.5%(3/40),the difference was statistically significant(x^2=60.709,68.396,P<0.05).In the observation group,the proportion of high expression of HIF-1αand PCDGF and the high expression of HIF-1αand PCDGF in T3,T4,III and IV lymph node metastasis were significantly higher than those in T1 and T2 stage.There was significant difference between stage I and II,no lymph node metastasis(P<0.05).Spearman correlation analysis showed that the expression of HIF-1αwas positively correlated with the expression of PCDGF in nasopharyngeal carcinoma(r=0.856,P<0.01).After 36 weeks of follow-up,the overall survival rate of the patients was 65.7%.The survival rates of patients with nasopharyngeal carcinoma were 87.5%,56.7%,67.9%,and 52.2,respectively.The survival rates of patients with nasopharyngeal carcinoma were significantly lower than those of patients with low expression of HIF-1αand PCDGF,high expression of HIF-1αand PCDGF,and high expression of HIF-1αand PCDGF.There was a statistically significant difference in survival between four groups(x^2=4.433,P<0.05).Conclusion:HIF-1αand PCDGF are highly expressed in patients with nasopharyngeal carcinoma,and the expression level is closely related to t
