非酒精性脂肪性肝炎患者血清CK18和FGF-21水平变化及临床意义探讨

摘　　要：目的探讨血清细胞角蛋白18(CK18)、成纤维细胞因子21(FGF-21)水平对非酒精性脂肪性肝炎的诊断价值。方法选取2019-01-01-2021-01-31朝阳市第四医院收治的120例非酒精性脂肪性肝炎患者作为研究对象,为观察组。另选取同院同期60位健康志愿者为对照组。比较2组CK18、FGF-21表达水平以及各生化指标水平。结果观察组CK18水平为(124.18±18.44)U/L,高于对照组的(73.27±12.46)U/L,F=19.284,P<0.001;FGF-21水平为(287.29±89.72)pg/mL,高于对照组的(137.65±54.81)pg/mL,F=11.851,P<0.001;随着病情的加重非酒精性脂肪性肝炎患者CK18、FGF-21水平逐渐升高,且差异有统计学意义,P<0.05。观察组AST水平为(38.48±8.73)U/L,高于对照组的(23.15±6.44)U/L,F=12.053,P<0.001;ALT水平为(45.28±8.77)U/L,高于对照组的(13.06±3.01)U/L,F=27.622,P<0.001;TC水平为(2.04±0.61)mmol/L,高于对照组的(0.95±0.21)mmol/L,F=13.432,P<0.001;TG水平为(4.78±0.74)mmol/L,与对照组的(4.67±0.82)mmol/L比较,差异无统计学意义,F=0.906,P=0.365;LDL水平为(2.91±0.74)mmol/L,与对照组的(2.87±0.63)mmol/L比较,差异无统计学意义,F=0.358,P=0.720;HDL水平为(1.01±0.42)mmol/L,低于对照组的(1.64±0.51)mmol/L,F=8.818,P<0.001。CK18和FGF-21水平与患者病情严重程度成正相关,r=0.879,P<0.001。结论CK18、FGF-21水平在非酒精性脂肪性肝炎患者血清中显著上升,且随着病情加重水平不断上升,这对非酒精性脂肪性肝炎的诊断具有一定参考价值。Objective To explore the diagnostic value of serum cytokeratin 18(CK18)and fibroblast growth factor 21(FGF-21)levels in nonalcoholic steatohepatitis.Methods Totally 120patients with nonalcoholic steatohepatitis treated Chaoyang Fourth Hospital from January 1,2019to January 31,2021were selected as the research objects and enrolled in the observation group and 60healthy people in the same period were enrolled in the control group.The expression levels of CK18and FGF-21and the levels of biochemical indexes were compared between the two groups.Results The level of CK18in the observation group(124.18±18.44)U/L was significantly higher than that in the control group(73.27±12.46)U/L,F=19.284,P<0.001.The level of FGF-21in the observation group(287.29±89.72)pg/ml was significantly higher than that in the control group(137.65±54.81)pg/ml,F=11.851,P<0.001.With the aggravation of the disease,the levels of CK18and FGF-21in patients with nonalcoholic steatohepatitis increased gradually,and the difference was statistically significant(P<0.05).The level of AST in the observation group(38.48±8.73)U/L was significantly higher than that in the control group(23.15±6.44)U/L,F=12.053,P<0.001.The ALT level in the observation group(45.28±8.77)U/L was significantly higher than that in the control group(13.06±3.01)U/L,F=27.622,P<0.001.The level of TC in the observation group(2.04±0.61)mmol/L was significantly higher than that in the control group(0.95±0.21)mmol/L,F=13.432,P<0.001.There was no significant difference in TG level between the observation group(4.78±0.74)mmol/L and the control group(4.67±0.82)mmol/L,F=0.906,P=0.365.There was no significant difference in LDL level between the observation group(2.91±0.74)mmol/L and the control group(2.87±0.63)mmol/L,F=0.358,P=0.720.The HDL level in the observation group(1.01±0.42)mmol/L was lower than that in the control group(1.64±0.51)mmol/L,F=8.818,P<0.001.The levels of CK18and FGF-21were positively correlated with the severity of the disease,r=0.879,P<0.001.Conclusion The ser
