摘 要:目的A超和光学相干生物测量仪(IOL Master)是近年来眼科应用较多的眼轴长度(axial length,AL)测量方法,但效果不一。本研究比较两种方法对于AL测量准确度与一致性,为白内障手术顺利进行提供依据。方法选择本院2017-01-02-2019-02-15诊治的白内障患者95例(140眼)作为研究对象,均在本院接受超声乳化以及手术治疗,术前分别采用A超以及IOL Master检测AL。根据IOL Master检测的AL值将患者分为A组(≤22 mm,26眼)、B组(>22~24mm,50眼)、C组(>24~26mm,23眼)、D组(>26~28mm,21眼)以及E组(>28mm,20眼),比较A超及IOL Master测量AL、平均绝对屈光误差值(mean absolute refractive error,MAE)和一致性。结果E组IOL Master检测AL值为(31.02±1.42)mm,低于A超检测的(30.23±1.29)mm,t=2.010,P=0.041。E组IOL Master检测MAE值为(0.65±0.23)D,低于A超检测的(0.87±0.36)D,t=2.626,P=0.011。两种检测方法在A、B、C、D组中分别有3.85%(1/26)、4.00%(2/50)、0(0/23)和0(0/21)的点在一致性界限外,均满足>95%的点在一致性界限内,具有较高的一致性,P>0.05;E组有10.00%(2/20)的点在一致性界限外,结果一致性较差,P<0.05。结论IOL Master在眼生物学测量中具有较为明显优势,与A超检测结果具有高度一致性,但是对于高度近视眼患者,其测量准确性更高。OBJECTIVE A-ultrasonic and optical coherent biological measuring instrument(IOL Master)is an eye axial length(AL)measurement method that has been widely used in ophthalmology in recent years,but the effect is different.This study compares the accuracy and consistency of the two methods for AL measurement and provides a basis for the smooth operation of cataract surgery.METHODS A total of 95 cases(140 eyes)of cataract patients diagnosed and treated in our hospital from January 2,2017 to February 15,2019 were selected as the research subjects.All of them received phacoemulsification and surgical treatment in our hospital.A ultrasonography and IOL Master were used to detect AL before surgery.According to the AL detected by the IOL Master,the patients were divided into group A(≤22 mm,26 eyes),group B(>22-24 mm,50 eyes),group C(>24-26 mm,23 eyes),and group D(>26-28 mm,21 eyes)and group E(>28 mm,20 eyes).AL,mean absolute refractive error(MAE)and consistency were measured by A-mode and IOL Master.RESULTS The AL value of the IOL Master test in group E was(31.02±1.42)mm,which was lower than that of the A-ultrasound test(30.23±1.29)mm,t=2.010,P=0.041.The MAE value of the IOL Master in group E was(0.65±0.23)D,which was lower than that of the A-ultrasound test(0.87±0.36)D,t=2.626,P=0.011.The two detection methods in the A,B,C and D groups were 3.85%(1/26),4.00%(2/50),0(0/23)and 0(0/21)outside the consistency limit,all points satisfying>95%were within the consistency limit,with high consistency,P>0.05.Group E has10.00%(2/20)points outside the consistency limit,the result was consistent poor,P<0.05.CONCLUSION IOL Master had obvious advantages in ophthalmology measurement,and it had high consistency with A-ultrasound results,but for patients with high myopia,the measurement accuracy was higher.
