摘　　要：阿尔茨海默病(Alzheimer diseases,AD)是导致痴呆症的最常见原因。随着人口老龄化问题日益加剧,AD的发病人数和导致的社会负担逐年上升。流行病学研究表明,AD与2型糖尿病(type 2 diabetes mellitus,T2DM)的发病具有显著相关性,其机制主要可以解释为胰岛素抵抗、胰岛素缺乏和氧化应激与β淀粉样蛋白(amyloidβ,Aβ)沉积和tau蛋白磷酸化相互作用,促进了AD的发生和发展。由此推测抗糖尿病药物或许对AD具有一定的治疗作用。本文选取近年来国内外关于AD与T2DM共病的发生机制及抗糖尿病药物治疗AD的疗效评价的研究,旨在为AD的治疗和新药开发提供新的思路和方向。在目前的临床研究中,多种抗糖尿病药物,包括胰岛素和胰岛素类似物、二甲双胍和噻唑烷二酮类化合物等均不同程度地显示出对AD的有益影响,抗糖尿病药物治疗AD的潜力已初步体现,其疗效评价和分子机制仍有待于进一步研究。Alzheimer’s disease(AD)is the most common cause of dementia worldwide.With the increasing aging of the population,the number of cases of AD and the social burden caused by AD are increasing year by year.Epidemiological studies have shown that there is a significant correlation between Alzheimer’s disease and type 2 diabetes mellitus(T2 DM).The mechanism can be explained by the interaction between insulin resistance(IR),insulin deficiency,oxidative stress and amyloid-β(Aβ)deposition and tau protein phosphorylation,which promote the occurrence and development of AD.Therefore,it is reasonable to speculate that antidiabetic drugs may have a certain therapeutic effect on AD.This article reviews the studies on the mechanism of AD and T2 DM co-disease and the evaluation of the efficacy of antidiabetic drugs in the treatment of AD in recent years,in order to provide new ideas and directions for the treatment and the development of new drugs of AD.In the current clinical studies,a variety of antidiabetic drugs,including insulin and insulin analogues,metformin and thiazolidinediones,have shown beneficial effects on AD in varying degrees.The potential of antidiabetic drugs in the treatment of AD has been initially reflected,and its efficacy evaluation and molecular mechanism still need to be further studied.
