摘　　要：目的探讨继发性肥大性下橄榄核变性(HOD)临床及影像学特点,提高临床识别及诊断率。方法回顾分析2019年4月至2020年1月收治的4例HOD患者临床资料,归纳临床、影像特点,探讨其发生机制。结果4例患者均为继发性HOD,脑干、小脑不同病变在锥体系症状好转的基础上出现锥体外系症状。磁共振(MRI)T成像示下橄榄核局限性高信号伴体积增大。结论脑干、小脑病变可导致继发性肥大性HOD。当锥体系症状平稳时出现锥体外系症状,需考虑HOD,结合影像检查可明确诊断。Objective To explore the clinical and imaging characteristics of secondary hypertrophic olivary degeneration(HOD)so as to improve the clinical recognition and diagnosis.Methods The clinical and imaging data of 4 patients treated during Apr.2019 and Jan.2020 were retrospectively analyzed,and the mechanism was discussed.Results All cases were secondary HOD.Symptoms of extra-vertebral system appeared,while symptoms of pyramidal system in different lesions of the brainstem and cerebellum were stable.Limited high signal was observed in the enlarged inferior olivary nucleus on TWI imaging.Conclusion Variant lesions in brain stem or cerebellum may lead to the occurrence of secondary HOD.When extra-vertebral system symptoms appear in addition to stable vertebral system symptoms,HOD should be considered.A clear diagnosis requires appropriate imaging examinations.
