黄芪防己汤加减治疗喉癌术后放疗并发淋巴水肿的疗效及对免疫功能和患者生活质量的影响

摘　　要：目的:探讨黄芪防己汤加减治疗喉癌术后放疗并发淋巴水肿的疗效及对免疫功能和患者生活质量的影响。方法:选择2018年6月~2020年10月包头市第四医院和包头市肿瘤医院共同收治的80例喉癌术后放疗并发淋巴水肿患者,根据治疗方式不同将患者均分为对照组和观察组。对照组给予压力治疗、手法淋巴引流、功能锻炼等常规消肿治疗2个月,观察组在对照组基础上增加黄芪防己汤加减治疗2个月。比较两组疗效和安全性,治疗前后水肿情况、免疫功能以及生活质量情况。结果:观察组治疗有效率为87.50%,高于对照组的67.50%(P<0.05),两组治疗后LENT-SOMA评分、改良Miller评分、CD8^(+)均降低(P<0.05),CD3^(+)、CD4^(+)、CD3^(-)CD16^(+)CD56^(+)水平、SF-36各项评分和总分均升高(P<0.05)。观察组治疗后LENT-SOMA评分、改良Miller评分、CD8^(+)低于对照组(P<0.05),CD3^(+)、CD4^(+)、CD3^(-)CD16^(+)CD56^(+)水平、SF-36各项评分和总分高于对照组(P<0.05)。结论:黄芪防己汤加减可明显减轻喉癌术后放疗并发淋巴水肿程度,提高免疫功能和生活质量。Objective To investigate the therapeutic effect of Huangqi Fangji Decoction on postoperative radiotherapy complicated with lymphedema in patients with laryngeal cancer and its influence on immune function and quality of life.Methods:80 cases of laryngeal cancer patients with postoperative radiotherapy complicated with lymphedema admitted to Baotou fourth hospital and Baotou Cancer hospital from June 2018 to October 2020 were selected and divided into control group and observation group according to different treatment methods.The control group was given pressure treatment,manual lymphatic drainage,functional exercise and other routine swelling treatment for 2 months,while the observation group was treated with modified Huangqi Fangji Decoction for 2 months on the basis of the control group.Before and after treatment,the efficacy and safety,edema,immune function and poor quality of life were compared between the two groups.Results The effective rate of the observation group was 87.50%,higher than 67.50%that of the control group(P<0.05).The LENT-SOMA score,modified Miller score and CD8^(+)were reduced in both groups after treatment(P<0.05),and levels of CD3^(+),CD4^(+),CD3^(-)CD16^(+)CD56^(+),and SF-36 scores and total scores were increased(P<0.05).After treatment,LENT-SOMA score,modified Miller score and CD8^(+)in the observation group were lower than those in the control group(P<0.05),CD3^(+),CD4^(+),CD3^(-)CD16^(+)CD56^(+)levels,and SF-36 scores and total scores were higher than those in the control group(P<0.05).Conclusion Huangqi Fangji Decoction can significantly reduce the degree of lymphedema after radiotherapy for laryngeal cancer,improve the immune function and quality of life.
