龙胆泄肝汤合八正散联合雄激素阻断治疗晚期前列腺癌(湿热蕴结型)的效果及对肿瘤标志物的影响

摘　　要：目的观察龙胆泄肝汤合八正散联合雄激素阻断治疗晚期前列腺癌(湿热蕴结型)的效果及对肿瘤标志物的影响。方法选取2019年5月~2020年5月收治的91例晚期前列腺癌(湿热蕴结型)患者为研究对象,根据随机数字表法分为对照组(45例)和观察组(46例),对照组给予雄激素阻断治疗,观察组给予龙胆泄肝汤合八正散联合雄激素阻断治疗,比较两组临床疗效,中医证候积分、恢复情况、肿瘤标志物水平及生活质量变化。结果观察组治疗后临床有效率显著高于对照组(P<0.05);观察组治疗后排尿苦难、尿淋漓不尽、腰痛腰酸、小腹胀满症候积分均明显低于对照组(P<0.05);观察组治疗后恢复较好者为73.91%,高于对照组的37.78%,差异有统计学意义(P<0.05);观察组治疗后血清癌胚抗原(CEA)、肿瘤细胞角蛋白19片段(CYFRA21~1)、神经元特异性烯醇化酶(NSE)水平均明显低于对照组(P<0.05);观察组生活质量测定量表(QLQ-C30)评分中症状领域、功能、总健康情况、单项条目评分均明显高于对照组(P<0.05)。结论龙胆泄肝汤合八正散联合雄激素阻断治疗晚期前列腺癌(湿热蕴结型)的效果显著,可改善临床症状,降低肿瘤标志物水平,提升生活质量。Objective To observe the effects of Longdan Xiegan Decoction and Bazheng San combined with androgen blockade in the treatment of advanced prostate cancer(dampness-heat accumulation type)and the influence on tumor markers.Methods From May 2019 to May 2020,91 patients with advanced prostate cancer(dampness-heat accumulation type)were selected and divided into control group(45 cases)and experimental group(46 cases)by the random number table method.The control group received androgen blockade therapy,while the experimental group were treated with Longdan Xiegan Decoction and Bazheng Powder combined with androgen blockade therapy.The clinical effects,changes in TCM syndrome scores,recovery,tumor markers and quality of life were compared between the two groups.Results The clinical response rate of the experimental group was significantly higher than that of the control group(P<0.05).After treatment,the scores for dysuria,incomplete emptying,low back pain and abdominal distension of the experimental group were significantly lower than those of the control group(P<0.05).The good recovery rate in the experimental group was 73.91%,higher than 37.78%in the control group(P<0.05).After treatment,serum carcinoembryonic antigen(CEA),cytokeratin 19 fragment(CYFRA21~1)and neuron-specific enolase(NSE)levels in the experimental group were significantly lower than those in the control group(P<0.05).The scores for symptoms,function,and overall health in the Quality of Life Questionnaire(QLQ-C30)of the experimental group were significantly higher than those of the control group(P<0.05).Conclusion Longdan Xiegan Decoction and Bazheng Powder combined with androgen blockade is effective in the treatment of advanced prostate cancer(dampness-heat accumulation type),which can improve clinical symptoms,lower the levels of tumor markers,and improve the patients’quality of life.
