摘  要:目的探讨伊曲康唑联合氟康唑对眼外伤后真菌感染疗效及炎症因子的影响。方法选择2016年1月-2018年1月公安县人民医院接诊的眼外伤后真菌感染患者67例作为研究对象,以随机数表法分为实验组(n=34)和对照组(n=33)。对照组使用氟康唑治疗,实验组在对照组的基础上加用伊曲康唑进行治疗。比较两组临床疗效,肿瘤坏死因子-α(TNF-α)、白细胞介素-4(IL-4)、白细胞介素-10(IL-10)、白细胞介素-12(IL-12)水平,临床症状改善时间及不良反应发生情况。结果实验组总有效率为91.18%(31/34),高于对照组的72.73%(24/33),差异有统计学意义(P<0.05)。实验组TNF-α、IL-12水平分别为(1.62±0.50)pg/L、(6.84±2.34)ng/L,均明显低于对照组(2.74±0.95)pg/L、(13.56±3.63)ng/L(t=6.064、9.034,P均<0.05);IL-4、IL-10水平分别为(3.05±1.33)ng/mL、(5.06±1.67)pg/mL,均明显高于对照组(2.34±0.85)ng/mL、(3.25±1.46)pg/mL,差异有统计学意义(t=2.595、4.717,P均<0.05)。治疗后,实验组临床症状改善时间均显著早于对照组,差异有统计学意义(P<0.05)。两组不良反应比较差异无统计学意义(P>0.05)。结论在眼外伤后真菌感染患者中使用伊曲康唑联合氟康唑效果显著,可有效改善患者炎症因子水平,值得推广与运用。Objective To study the effect of itraconazole combined with fluconazole on fungal infection and the induced inflammatory factors after ocular trauma.Methods A total of 67 patients with fungal infection after ocular trauma admitted in our hospital from January 2016 to January 2018 were selected as the research objects,and were divided into experimental group(n=34)and control group(n=33)by random number table method.The control group was treated with fluconazole,and the experimental group was treated with itraconazole on the basis of the control group.Clinical efficacy,tumor necrosis factor-α(TNF-α),interleukin-4(IL-4),interleukin-10(IL-10),interleukin-12(IL-12),improvement time of clinical symptoms and occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the experimental group(91.18%)was significantly higher than that of the control group(72.73%)(P<0.05).The levels of TNF-αand IL-12 in experimental group(1.62±0.50)pg/L,(6.84±2.34)ng/L were significantly lower than those in control group(2.74±0.95)pg/L,(13.56±3.63)ng/L(t=6.064,9.034,all P<0.05).The levels of IL-4 and IL-10(3.05±1.33)ng/m L,(5.06±1.67)pg/m L were significantly higher than those of the control group(2.34±0.85)ng/m L,(3.25±1.46)pg/m L(t=2.595,4.717,all P<0.05).After treatment,the improvement time of clinical symptoms in the experimental group was significantly earlier than that in the control group(P<0.05).There was no significance difference in adverse reactions between the two groups(P>0.05).Conclusion s Itraconazole combined with fluconazole had a significant effect on patients with fungal infection after eye trauma,which could effectively improve the level of inflammatory factors in patients and reduce complications.Therefore,itraconazole should be worth of promotion and application.
