发作性睡病145例的临床特征分析

摘　　要：目的通过对145例发作性睡病(NRL)患者的临床特征进行分析,研究中国人NRL的典型临床表现和非典型临床表现,为临床诊治提供重要依据。方法统计自2017年3月至2019年1月于首都医科大学宣武医院神经内科睡眠门诊就诊,并确诊为NRL的145例患者的睡眠障碍临床特征问卷,分析NRL患者的临床特征。结果145例NRL患者中,其性别比例、起病年龄分布以及典型的临床表现四联症的发生率与国外报道基本一致。而非典型临床表现中,失眠、噩梦、快速动眼期睡眠行为异常(RBD)的发生率远高于国外报道。本研究还发现,情绪障碍也是NRL常见的伴发疾病。结论NRL除了典型的四联症外,还有很多其它非典型临床症状,临床上需要全面分析这些临床症状,进行综合的治疗。Objective We studied the typical and atypical clinical manifestations of narcolepsy(NRL)in China by analyzing the clinical characteristics of 145 patients with narcolepsy NRL.We aimed to provide important basis for clinical diagnosis and treatment of NRL.Methods We investigatedthe questionnaires of clinical characteristics of sleep disordersfrom145 patients with NRL who visited Neurology Department of our hospital from March 2017 to January 2019,and analyzed the clinical characteristics of NRL.Results We found that the sex ratio,age distribution of disease onset and the incidence of four typical clinical manifestations in the 145 patients were similar to the previous NRL.Nevertheless,the incidence of some atypical clinical manifestations,including insomnia,nightmare and REM sleep behavior disorder(RBD),were much higher than the foreign reports.In our study,we also found thatemotional disorders were common concomitant disease of NRL.Conclusion In addition to the four typical clinical manifestations of NRL,there were some atypical clinical manifestations,which need to be analyzed thoroughlyin order to give comprehensive treatment.
