针刺结合物理康复训练治疗对脑瘫儿童神经电生理和运动障碍的影响

摘　　要：目的探究针刺结合物理康复训练治疗对脑瘫(CP)儿童神经电生理和运动障碍的影响。方法选取2019年6月—2020年12月医院收治的CP患儿60例,按照随机数字表法将其均分观察组(n=30)和对照组(n=30),观察组行针刺结合物理康复训练治疗,对照组行物理康复训练治疗,比较两组临床疗效,采用粗大运动功能量表88项(GMFM-88)、改良Ashworth标准及精细运动功能量表(FMFM)评估两组运动及肌肉痉挛情况,观察两组视觉诱发电位(VEP)P100的振幅和潜伏期,比较两组干预前后血管内皮生长因子(VEGF)、肌酸激酶脑型同工酶(CK-BB)及神经元特异性烯醇化酶(NSE)水平及生活质量评分。结果观察组治疗总有效率(96.67%)显著高于对照组(76.67%)(P<0.05),治疗后,观察组VEP P100振幅、潜伏期、GMFM-88评分、改良Ashworth评分、FMFM评分、VEGF水平、CK-BB水平、NSE水平及生活质量评分均显著优于对照组。结论针刺结合物理康复训练治疗CP患儿能有效提升患儿的运动功能,缓解其肌肉痉挛情况,改善其神经电生理及生活质量。Objective To explore the effects of acupuncture combined with physical rehabilitation training on neuroelectrophysiology and dyskinesia in children with cerebral palsy(CP).Methods A total of 60 CP children admitted to the hospital from June 2019 to December 2020 were enrolled and divided into observation group(n=30)and control group(n=30)according to random number table method.The observation group was given acupuncture combined with physical rehabilitation training,while the control group was given physical rehabilitation training.The clinical curative effect was compared between the two groups.The exercise and muscle spasm in both groups were evaluated by 88-item Gross Motor Function Measure(GMFM-88),modified Ashworth Standard and Fine Motor Function Measure(FMFM).The amplitude and latency of visual evoked potential(VEP)P100 in both groups were observed.The levels of vascular endothelial growth factor(VEGF),creatine phosphokinase BB isoenzyme(CK-BB)and neuron specific enolase(NSE),and quality of life score before and after intervention were compared between the two groups.Results The total response rate in the observation group was significantly higher than that in the control group(96.67%vs 76.67%)(P<0.05).After treatment,amplitude and latency of VEP P100,scores of GMFM-88,modified Ashworth and FMFM,levels of VEGF,CK-BB and NSE,and quality of life in the observation group were significantly better than those in the control group.Conclusion Acupuncture combined with physical rehabilitation training can effectively improve motor function of CP children,relieve muscle spasms and improve neuroelectrophysiology and quality of life.
