摘  要:目的检测核因子红细胞2相关因子2(Nrf2)、低氧诱导因子-1α(HIF-1α)在食管鳞癌组织及癌旁组织中的表达水平并分析临床病理意义,初步探索Nrf2及HIF-1α在食管鳞癌发生发展过程中的作用机制。方法收集2010年6月~2020年8月湖北省肿瘤医院病理科食管鳞癌组织及对应癌旁组织切片各34例,采用免疫组化分析食管鳞癌组织及癌旁组织的Nrf2、HIF-1α、钙黏蛋白E(E-cadherin)及鼠双微体基因(mdm2)的阳性率。采用Graphpad Prism5.0软件分析Nrf2、HIF-1α不同的表达水平对患者术后无病进展生存期(PFS)的影响。结果Nrf2及HIF-1α在食管鳞癌组织中呈高表达并均与肿瘤浸润深度、T分期及有无淋巴结转移密切相关。Nrf2、HIF-1α及mdm2在食管鳞癌组织的表达阳性率均高于癌旁组织(P<0.05),Ecadherin在食管鳞癌组织的表达阳性率低于癌旁组织(P<0.05)。食管鳞癌组织中Nrf2的表达与HIF-1α、mdm2表达呈正相关,与E-cadherin表达呈负相关(P<0.05)。Nrf2和HIF-1α的不同表达水平患者术后PFS比较差异均无统计学意义(P>0.05)。结论Nrf2、HIF-1α在食管鳞癌中高表达,可能通过Nrf2/HIF-1α信号通路下调E-cadherin及上调mdm2促进食管鳞癌发生和发展。Objective To detect the expression and clinicopathological significance of Nuclear factor erythroid 2-related factor 2(Nrf2)and Hypoxia inducible factor-1α(HIF-1α)in esophageal squamous cell carcinoma(ESCC)and adjacent tissues,and to explore the mechanism of Nrf2 and HIF-1αin the occurrence and development of ESCC.Methods From June 2010 to August 2020,34 cases of esophageal squamous cell carcinoma tissues and corresponding adjacent tissues were collected in the Department of Pathology of Hubei Cancer Hospital.The ESCC and its corresponding adjacent tissues were processed by immunohistochemistry.Expressions of Nrf2,HIF-1α,E-cadherin and mdm2 were analyzed.Graphpad Prism5.0 software was used to analyze the effects of different expression states of Nrf2 and HIF-1αon postoperative Progression-free survival(PFS).Results Nrf2 and HIF-1αare highly expressed in ESCC and are closely related to the depth of tumor invasion,T stage and lymph node metastasis.The positive rate of Nrf2、HIF-1αand mdm2 expression in ESCC were all higher than those in adjacent tissues(P<0.05),the positive rates of Ecadherin expression in ESCC was lower than that in adjacent tissues(P<0.05).The expression of Nrf2 was positively correlated with the expression of HIF-1αand mdm2 in ESCC,and it was negatively correlated with E-caherin expression.There was no significant difference in postoperative PFS between patients with different expression level of Nrf2 and HIF-1α.Conclusion Nrf2 and HIF-1αare highly expressed in ESCC,which may promote the occurrence and development of ESCC by down regulating E-cadherin and up regulating mdm2 through Nrf2/HIF-1αsignaling pathway.
