单根锉根管预备器械对根管治疗术后疼痛影响的临床研究

摘　　要：目的:研究单根锉根管预备系统对于根管治疗术后疼痛的发生、程度以及持续时间的影响。方法:选择需要双侧同名磨牙需要进行根管治疗的患者96名,采用自身对照的方法,同一患者双侧磨牙分别接受Reciproc单根锉系统(实验组)和Mtwo根管预备系统(对照组)进行根管预备。利用问卷调查记录患者术后疼痛是否发生,疼痛发生持续时间以及疼痛的患者视觉模拟评分(visual analogue scale,VAS)自评分值。结果:术后对照组未发生疼痛的人数要多于实验组(χ~2=5.658,P=0.017),而发生严重疼痛的人数要少于实验组(χ~2=4.043,P=0.04)。实验组的VAS自评分值要高于对照组(P=0.000)。两组之间疼痛持续时间差异无统计学意义。结论:Reciproc单根锉根管预备系统相较于Mtwo连续旋转根管预备系统更易发生疼痛,并且疼痛的程度更高。Objective:To assess the influence of the single-file Reciprocating systems on incidence,intensity,and duration postoperative pain(PP).Methods:Root canal treatments of 96 patients were carried out with Reciproc systems and Mtwo rotary systems.A self-compared method was used.Each patient first received the operation with Mtwo rotary systems as the controlled group.In the second operation(experimental group),the contralateral same name molar was treated with singlefile Reciprocating systems.Patients were given a questionnaire to record the presence or absence of post-endodontic pain,its duration and level of discomfort.Results:Compared with patients who developed no pain,the control group showed significantly better results(χ2=5.658,P=0.017).When evaluating patients experiencing severe pain,the incidence of symptoms was significantly higher with the Reciproc technique(χ2=4.043,P=0.04).The values of visual analogue scale(VAS)in control group were lower than the experimental group(P=0.000).Differences in pain duration were not statistically significant between two groups.Conclusion:The results of this prospective in vivo study suggest that a higher incidence and level of PP should be expected after root canal preparation with single-file Reciprocating systems.
