摘　　要：目的探讨紧张型头痛(TTH)患者大脑灰质体积的改变情况及其与疼痛程度的相关性。方法对2018年5月至2019年7月到潍坊医学院附属医院就诊的TTH患者33例(TTH组)及与之匹配的健康志愿者38名(对照组)行全脑3D-T1WI结构像扫描,采用VBM-DARTEL的方法分析两组灰质体积的差异,进一步分析差异脑区与疼痛程度的相关性。结果与对照组相比,TTH组左眶部额下回、右眶部额中回、左侧枕中回、左侧小脑下部、右侧海马、右侧楔前叶、右侧舌回灰质体积减少(P<0.001,FWEc校正),未见灰质体积增加脑区;差异脑区与疼痛程度无相关性。结论TTH患者存在多个灰质体积减少的脑区,这些脑区可能参与了TTH的中枢病理生理机制。Objective Using voxel-based morphometry to investigate the changes of gray matter volume in patients with tension-type headache and its correlation with the degree of pain.Methods 33 cases of tension-type headache patients and 38 cases of matched healthy control volunteers underwent whole brain 3 D-T1WI structural phase imaging from May 2018 to July 2019 in Affiliated Hospital of Weifang Medical University.VBM-DARTEL method was used to analyze the difference of gray matter volume between the two groups,and further analyze the correlation between different brain regions and pain degree.Results The volume of gray matter in the left orbital inferior frontal gyrus,right orbital middle frontal gyrus,left occipital middle gyrus,left cerebellum7 b,right hippocampus,right precuneus and right lingual gyrus,decreased in the tension headache group compared with the healthy control group(P<0.001,FWE cluster corrected).No increased gray matter volume was shown between the two groups.There was no correlation between the decreased brain areas and the pain degree.Conclusion Tension-type headaches had multiple brain regions with decreased gray matter volume.These brain regions may be involved in the central pathophysiological mechanism of tension-type headache.
