阴道斜隔综合征的MRI诊断价值探讨

摘　　要：目的探讨阴道斜隔综合征的MRI表现及诊断价值。方法回顾性分析经手术确诊的12例阴道斜隔综合征患者的临床及MRI影像学资料,评价MRI所示子宫及阴道情况、斜隔位置及走行、有无瘘口及伴发泌尿系统异常情况,与术中所见对照分析。结果12例患者中,Ⅰ型5例,平均年龄12岁;Ⅱ型4例,平均年龄23.7岁;Ⅲ型3例,平均年龄24岁。5例Ⅰ型患者均以痛经就诊,7例Ⅱ型及Ⅲ型患者以痛经、不孕或不规则阴道出血就诊。MRI检查12例均可见双子宫、双阴道及阴道斜隔;左侧斜隔7例,右侧斜隔5例;伴发泌尿系统畸形者11例,无显著泌尿系异常1例。结论阴道斜隔综合征临床症状不具特异性,MRI可较清晰显示病变结构,有助于明确诊断及分型;对双子宫病例,需警惕本病可能。Objective To analyze imaging manifestations and diagnostic value of contrast-enhanced MRI in oblique vaginal septum syndrome(OVSS).Methods The clinical and MR imaging data of 12 patients with OVSS diagnosed by surgery in our hospital during 2016 and 2019 were retrospectively analyzed.Image evaluation included uterine and vaginal conditions,oblique septum position and alignment,presence of fistula and urinary system abnormalities,which were compared with surgical findings.Results Among 12 patients,5 patients with median age of 12 years were typeⅠ,4 patients with median age of 23.7 years were typeⅡ,3 patients with median age of 24 years were typeⅢ.5 patients with typeⅠpresented with dysmenorrhea,and 7 patients with typeⅡandⅢpresented with dysmenorrhea,infertility or irregular vaginal bleeding.Duplicated uterus,duplicated vagina,and vaginal oblique septum were found on MRI examinations;7 had left oblique septum and 5 cases had right oblique septum;11 cases had system abnormalities and 1 case without system abnormalities.Conclusion The clinical symptoms of OVSS are not specific.MR imaging demonstrates abnormal structures and helps in diagnosis and classification.In cases of duplicated uterus,we need to be alert to the possibility of OVSS.
