摘 要:目的探讨细针辅助穿刺技术在兔肺肿瘤病灶消融穿刺中的实用性及优越性。方法30只实验兔CT引导下,穿刺方式建立兔肺VX2肿瘤模型。术后3周经CT确定模型建立成功后,将模型兔随机分为实验组及对照组,并进行消融针对比穿刺实验。其中实验组模型兔在CT引导下先于右侧胸壁采用细针穿刺至病灶中心,后采用消融针于兔前胸壁在CT引导下穿刺病灶中心;对照组模型兔采用消融针直接穿刺兔肺病灶。分别记录两组穿刺操作时间、成功率、穿刺次数、气胸、肺部出血情况,并比较两者之间的差异。结果30只实验兔均建模成功,术后3周CT扫描均可见右下肺内类圆形病灶。实验组穿刺成功率为93.3%,对照组为33.3%(P=0.001);实验组穿刺操作时间为(16.05±4.38)分,对照组为(28.63±4.19)分(P<0.001);实验组气胸发生率为40%,对照组为80%(P=0.025);实验组肺实质出血概率为53.3%,对照组为80%(P=0.121);实验组平均穿刺次数为(4.40±1.50)次,对照组为(6.87±1.92)次(P=0.01)。结论细针辅助穿刺技术能有效降低兔肺病灶消融穿刺的难度,缩短穿刺操作时间,降低气胸及肺出血发生率,提高穿刺成功率。Objective To explore the feasibility and superiority of fine needle assisted puncture technique in ablation and puncture of rabbit lung tumor lesions.Methods 30 experimental rabbits were punctured under CT guidance to establish rabbit lung VX2 tumor models.Three weeks after the operation,CT confirmed that the model was successfully established.The model rabbits were randomly divided into an experimental group and a control group,and ablation-specific puncture experiment was performed.The experimental group model rabbits were punctured to the center of the lesion with a fine needle through the right chest wall under CT guidance,and then an ablation needle was used to puncture the center of the lesion under the guidance of CT in the anterior chest wall of the rabbit.The rabbits in the control group were directly punctured with ablation needles to the lung lesions.We recorded the puncture procedure time,success rate,number of punctures,pneumothorax,and pulmonary hemorrhage in the two groups respectively,and compared the differences between the two.Results All 30 experimental rabbits were successfully modeled,and the CT scan showed round lesions in the right lower lung 3 weeks after operation.The puncture success rate of the experimental group was 93.3%,and the control group was 33.3%(P=0.001).The puncture procedure time of the experimental group was 16.05±4.38 minutes,and the control group was 28.63±4.19 minutes(P<0.001).The incidence of pneumothorax in the experimental group was 40%,and 80%in the control group(P=0.025).The probability of pulmonary hemorrhage in the experimental group was 53.3%,and 80%in the control group(P=0.121).The average number of punctures in the experimental group was 4.40±1.50,and the control group was 6.87±1.92(P=0.01).Conclusion The fine needle assisted puncture technique can effectively reduce the difficulty of ablation and puncture of rabbit lung lesions,shorten the puncture procedure time,reduce the incidence of pneumothorax and pulmonary hemorrhage,and increase the success rate of
