CT引导下125I粒子植入术联合PC化疗对Ⅲa期非小细胞肺癌患者血清癌胚抗原、鳞状细胞癌抗原水平及生存质量的影响

摘　　要：目的探讨CT引导下125I粒子植入术联合紫杉醇+卡铂(PC)化疗对Ⅲa期非小细胞肺癌(NSCLC)患者血清癌胚抗原(CEA)、鳞状细胞癌抗原(SCCAg)水平及生存质量的影响。方法搜集2015年4月至2018年5月本院收治的54例Ⅲa期NSCLC患者,简单随机分为两组,每组各27例。对照组给予放疗与PC化疗,观察组给予CT引导下125I粒子植入术联合PC化疗,比较两组治疗后缓解率、疾病控制率、血清肿瘤标志物[CEA、细胞角蛋白21-1片段(CYFRA21-1)、SCCAg]水平、胰岛素样生长因子-1(IGF-1)、胰岛素样生长因子结合蛋白-3(IGFBP-3)、欧洲癌症研究治疗组织研发癌症患者生活质量评定量表(EORTC QLQ-C30)评分、不良反应。结果观察组治疗后缓解率、疾病控制率(70.37%、88.89%)高于对照组(37.04%、55.56%)(P<0.05);观察组治疗后血清CEA、CYFRA21-1、SCCAg水平低于对照组(P<0.05);观察组治疗后血清IGF-1低于对照组,IGFBP-3高于对照组(P<0.05);观察组治疗后整体生活质量量表、功能量表高于对照组,症状量表、单项测试项目评分低于对照组(P<0.05);两组胃肠道反应、骨髓抑制、脱发、气胸发生率比较差异无统计学意义(P>0.05)。结论CT引导下125I粒子植入术联合PC化疗治疗Ⅲa期NSCLC,可有效抑制肿瘤细胞增殖,提高缓解率、疾病控制率,降低血清肿瘤标志物水平,提高患者生存质量,安全性高。Objective To analyze the effect of 125I seed implantation guided by computed tomography(CT)combined with paclitaxel plus carboplatin(PC)chemotherapy on serum levels of carcinoembryonic antigen(CEA),squamous cell carcinoma antigen(SCCAg)and quality of life in patients with stageⅢa non-small cell lung cancer(NSCLC).Methods From April 2015 to May 2018,54 patients with stageⅢa SCLC admitted to our hospital were randomized into two groups,27 cases each.The control group received PC chemotherapy,while the observation group received CT-guided 125I seed implantation combined with PC chemotherapy.The remission rate,disease control rate,serum tumor markers[CEA,CYFRA21-1,SCCAg],insulin-like growth factor-1(IGF-1),insulin-like growth factor binding protein-3(IGFBP-3),quality of life scale score of European cancer research group(EORTC QLQ-C30)and the adverse reactions were compared.Results After treatment,the remission and disease control rates of the observation group(70.37%,88.89%)were higher than that of the control group(37.04%,55.56%)(P<0.05).The scale was higher than that of the control group,and the scores of symptoms scale and individual test items were lower than that of the control group(P<0.05).There was no significant difference in the incidence of gastrointestinal reactions,bone marrow suppression,alopecia and pneumothorax between the two groups(P>0.05).Conclusion CT-guided 125I seed implantation combined with PC chemotherapy in the treatment of stageⅢa NSCLC can effectively inhibit the proliferation of tumor cells,improve the remission rate and disease control rate,reduce the level of serum tumor markers,and improve the quality of life of patients with high safety.
