225例单纯HPV E6/E7 mRNA阳性者阴道镜检查及病理结果分析

摘　　要：目的分析不同亚型单纯HPV E6/E7 mRNA阳性者阴道镜检查及病理结果。方法前瞻性研究2018年1月—2020年5月于郑州大学第三附属医院进行宫颈细胞学及HPV E6/E7 mRNA联合筛查,细胞学正常,因HPV E6/E7 mRNA阳性而转诊阴道镜的患者225例,年龄25~65岁,分为16/18/45亚型阳性组与其他亚型阳性组,均进行阴道镜下宫颈四个象限活检及宫颈管搔刮。结果225例患者中,阴道镜印象HGL患者9.3%(21/225),病理HSIL+的检出率为24.0%(54/225),其中阴道镜印象HGL患者中HSIL检出率100%,(21/21),阴道镜印象LGL以及正常患者中HSIL检出率为16.2%(33/204)。16/18/45亚型阳性组与其他亚型阳性组高级别病变检出率差异无统计学意义。结论不同亚型单纯HPV E6/E7 mRNA阳性的患者HSIL检出率差别无统计学意义,建议非16/18/45亚型E6/E7 mRNA阳性患者转诊阴道镜检查。Objective To analyze the colposcopy and pathological results of different subtypes of HPV E6/E7 mRNA positive/cytology-negative patients.Methods This study enrolled 225 women aged 25~65 years who had underwent cervical screening by liquid-based cytology combined with HPV E6/E7 mRNA test at the Third Affiliated Hospital of Zhengzhou University between January 2018 to May 2020.They cytology-negative/HPV E6/E7 mRNA positive patients were referred to colposcopy.They were divided into 16/18/45 subtype positive group and other subtype positive group.All of them accepted 4-quadrant cervical biopsies with endocervical curettage(ECC)under colposcopy.Results The colposcopy impression of HGL was 9.3%(21/225).The detection rate of pathological HSIL+was 24.0%(54/225).There was no statistically significant difference in the detection rate of high-grade lesions between the two groups.Conclusion For HPV E6/E7 mRNA positive/cytology-negative patients,there was no statistically significant difference in HSIL detection rate among patients with different subtypes of HPV,it is recommended that HPV E6/E7 mRNA positive/cytology-negative patients should be referred for colposcopy.
