摘　　要：腰方肌阻滞(quadratus lumborum block,QLB)属于躯干神经阻滞,腹部手术的围术期镇痛在过去主要以硬膜外镇痛和静脉镇痛为主,但随着外科腔镜微创技术的发展,以及快速康复外科理念的要求,其临床使用逐渐减少,区域神经阻滞术后镇痛的应用也越来越多。Blanco[1]在2007年提出的腰方肌阻滞适用于腹部、髋部及下肢手术的围术期镇痛,研究表明,与腹横肌平面阻滞(transversus abdominis plane block,TAPB)比较,QLB能同时阻断体表躯体痛与内脏痛,镇痛效果更好、持续时间更长,成了近几年的研究热点。笔者就腰方肌阻滞用于腹部手术围术期镇痛研究进展作一综述。Quadratus lumborum block(QLB)belongs to the trunk nerve block,the perioperative analgesia of abdominal surgery mainly rely on epidural anesthesia and intravenous analgesia in the past,but with the development of minimally invasive surgical cavity mirror technology,and to meet the requirements of rapid rehabilitation surgery concept,its clinical use gradually reduce and regional nerve block is more and more applied to postoperative analgesia.In 2007,Blanco put forward that quadratus lumborum block was suitable for perioperative analgesia in the abdomen,hips and lower limb surgeries.Studies have shown that compared with transverse abdominal muscle plane block(TAPB),QLB can block surface body pain and visceral pain at the same time with better and longer analgesia effects,which has become the research focus in recent years.The authors review the research orogress of quadratus lumborum block used for Derioperative analgesia in abolominal surgery.
