大便隐血检测及血浆PT-INR在结直肠癌筛查中的应用

摘　　要：目的:探讨大便隐血检测及血浆凝血酶原时间-国际标准化比值(PT-INR)在结直肠癌筛查中的应用价值。方法:将我院2019-2021年收治的50例结直肠癌患者设为观察组,另将50名同期健康体检者设为对照组,2组均行大便隐血检测,比较检验结果,并收集2组相关资料比较PT-INR值。采用受试者工作特征曲线(ROC)、曲线下面积(AUC)及Z检验评价分析大便隐血检测及血浆PT-INR对结直肠癌的预测价值。结果:观察组患者大便隐血阳性检出率为76.00%(38/50),明显高于对照组的8.00%(4/50),2组比较差异有统计学意义(P<0.05);观察组患者血浆PT-INR显著高于对照组(P<0.05),经ROC曲线得出,单独大便隐血检测诊断结直肠癌的AUC为0.840,灵敏度为76.0%,特异度为92.0%,单独血浆PT-INR诊断结直肠癌的AUC为0.874,灵敏度为80.0%,特异度为88.0%,两者联合诊断结直肠癌的AUC为0.959,灵敏度为92.0%,特异度为94.0%。结论:大便隐血检测及血浆PT-INR联合预测结直肠癌灵敏度、特异度更高,可为疾病早期筛查提供参考依据。Objective To explore the application value of stool occult blood test(SOBT)and plasma PT-INR in screening colorectal cancer.Methods Enrolled 50 patients with colorectal cancer treated in author’s hosptial(2019-2021)into observation group,at the same time,50 physical examinees into control group,both all received SOBT;then,compared both results,and collected both relevant data so as to compare PT-INR value;afterwards,by using ROC,AUC and Ztest results so as to evaluate the value of SOBT and plasma PT-INR in prognosing colorectal cancer.Results In positive detected rate of SOBT[76.00%(38/50)vs 8.00%(4/50)]and in PT-INR observation group was all significantly higher than control group(all,P<0.05);in results from ROC including AUC,sensitivity,and specificity,respectively,1)alone SOBT:0.840,76.00%,and 92.00%;2)alone PT-INR:0.874,80.00%,and 88.00%;3)the combined detection of SOBT and PT-INR:0.959,92.0%,and 94.0%.Conclusion The combined detection of SOBT and PT-INR in the prognosticating colorectal cancer,has more higher sensitivity and specificity,thus can provide basis for early screening colorectal cancer.
