摘  要:目的研究皮肤鳞状细胞癌(CSCC)中乙酰肝素酶(HPA)与PI3K/AKT通路表达的相关性及临床意义。方法收集86例手术切除的CSCC组织及62例正常皮肤组织,采用免疫组化染色检测组织中HPA的阳性表达率,荧光定量聚合酶链反应(PCR)检测组织中HPA的mRNA表达水平,Western blot检测HPA、p-PI3K、p-AKT的蛋白表达水平。比较CSCC组织和正常皮肤组织中HPA表达水平的差异,采用Spearman及Pearson检验分析CSCC组织中HPA表达水平与p-PI3K、p-AKT表达水平的相关性。随访CSCC患者的预后,并绘制K-M曲线,采用log-rank检验分析不同HPA表达水平的CSCC患者预后的差异。结果CSCC组织中HPA的阳性表达率、mRNA表达水平、HPA蛋白表达水平及p-PI3K、p-AKT的蛋白表达水平均高于正常皮肤组织(P<0.05);CSCC组织中HPA的表达水平与p-PI3K、p-AKT的表达水平呈正相关(P<0.05);中低分化、有淋巴结转移的CSCC组织中HPA的阳性表达率、mRNA表达水平、蛋白表达水平均高于高分化、无淋巴结转移的CSCC组织(P<0.05);CSCC中HPA阳性表达、mRNA表达水平≥中位数、蛋白表达水平≥中位数患者的累积总生存率均低于HPA阴性表达、mRNA表达水平<中位数、蛋白表达水平<中位数患者(P<0.05)。结论CSCC组织中HPA表达增加与下游PI3K/AKT通路激活、分化程度降低、淋巴结转移及预后不良有关。Objective To study the correlation and clinical significance of heparanase(HPA)and PI3K/AKT pathway expression in cutaneous squamous cell carcinoma(CSCC).Methods Eighty-six cases of CSCC and sixty-two cases of normal skin tissues were collected.The positive expression rate of HPA was detected by immunohistochemistry.The mRNA expression level of HPA was detected by fluorescence quantitative PCR,and the protein expression levels of HPA,p-PI3K and p-AKT were detected by Western blot.The difference of HPA expression level between CSCC tissue and normal skin tissue was compared.The correlation between HPA expression and the expression levels of p-PI3K and p-AKT in CSCC tissue was analyzed by Spearman and Pearson test.The prognosis of CSCC patients was followed up and the K-M curve was drawn.The difference in prognosis of CSCC patients with different HPA expression levels was analyzed by log rank test.Results The positive expression rate,mRNA expression level,protein expression level of HPA and the protein expression levels of p-PI3K and p-AKT in CSCC were higher than those in normal skin tissue(P<0.05).The expression level of HPA in CSCC was positively correlated with the expression levels of p-PI3K and p-AKT(P<0.05).The positive expression rate,mRNA expression level and protein expression level of HPA in CSCC tissues with moderately differentiation,lymph node metastasis were higher than those in CSCC tissues with highly differentiated,non-lymph node metastasis(P<0.05).The cumulative overall survival rate of patients with HPA positive expression,mRNA expression level≥median and protein expression level≥median in CSCC was lower than that of patients with HPA negative expression,mRNA expression level<median and protein expression level<median(P<0.05).Conclusion The increased expression of HPA in CSCC is related to the activation of downstream PI3K/AKT pathway,the decrease of differentiation,lymph node metastasis and poor prognosis.
