摘 要:目的对比静态调强与容积旋转调强放射两种治疗技术对肺癌的治疗效果.方法于我院2018年12月至2019年12月收治的肺癌患者中抽取30例作为观察对象,分别应用静态调强与容积旋转调强放射治疗.结果肺癌患者静态调强(IMRT)治疗后,肺部V5与V10受量分别为(46.2±11.6)、(37.3±8.4),与容积旋转调强(VMAT)的(49.2±12.4)、(39.4±8.6)相比,差异显著,P<0.05;肺癌患者经静态调强(IMRT)与容积旋转调强(VMAT)治疗,其肺部V20、V30受量无差异,P>0.05.肺癌患者静态调强(IMRT)治疗后,心脏V20、V30及V40受量分别为(46.8±13.7)、(28.1±9.4)及(12.2±3.1),与容积旋转调强(VMAT)的(43.2±13.4)、(23.3±6.4)及(9.6±1.7)相比,差异显著,P<0.05.肺癌患者经静态调强(IMRT)放射治疗,PTV、CI及CI参数值分别为(6612.7±35.4)、(0.76±0.09)及(343.2±86.4),与容积旋转调强(VMAT)相比,存在明显差异性,P<0.05;而静态调强(IMRT)的HI(1.11±0.06),与容积旋转调强(VMAT)的(1.08±0.06)无明显差异性,P>0.05.结论静态调强与容积旋转调强放射治疗计划虽对患者肺癌靶区的剂量覆盖存在明显改善作用,但对患者的正常器官却缺乏保护作用,因此,心功能正常,但肺功能异常的患者,建议选择静态调强(IMRT)放射治疗,而肺功能正常,心功能异常的患者,建议选用容积旋转调强(VMAT)放射治疗.Objective To compare the therapeutic effects of static intensity-modulated radiation and volume rotation intensity-modulated radiation on lung cancer.Methods From December 2018 to December 2019,30 patients with lung cancer were selected as the observation objects,and were treated with static intensity modulation and volume rotation intensity modulation radiotherapy respectively.Results Compared with VMAT(49.2±12.4)and VMAT(39.4±8.6),there was no significant difference(P<0.05)between lung cancer patients treated with IMRT and VMAT.After IMRT treatment,the cardiac V20,V30 and V40 were(46.8±13.7),(28.1±9.4)and(12.2±3.1)respectively,which were significantly different from those of VMAT(43.2±13.4),(23.3±6.4)and(9.6±1.7),P<0.05.The parameters of PTV,CI and CI were(6612.7±35.4),(0.76±0.09)and(343.2±86.4)respectively,which were significantly different from that of VMAT(P<0.05),while hi of IMRT(1.11±0.06)was not significantly different from that of VMAT(1.08±0.06),P>0.05.Conclusion Although the static intensity modulation and volume rotation intensity modulation radiotherapy plan can significantly improve the dose coverage of lung cancer target area,it lacks the protective effect on the normal organs of patients.Therefore,for patients with normal heart function but abnormal lung function,it is recommended to choose static intensity modulation(IMRT)radiotherapy,while for patients with normal lung function and abnormal heart function,it is recommended to choose volume rotation intensity modulation Strong(VMAT)radiotherapy.
