草酸艾司西酞普兰与舍曲林治疗产后抑郁症的对照研究

摘　　要：目的 对产后抑郁症患者使用草酸艾司西酞普兰、舍曲林治疗的效果进行对照研究.方法 共入选产后抑郁症患者60例,均符合CCMD-3抑郁发作诊断标准,分为草酸艾司西酞普兰组(用药剂量10-20mg/d)、舍曲林组(用药剂量50-100mg/d).分别使用汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)评定治疗效果,用TESS和实验室检查评定治疗安全性.两组均接受为期6周的治疗观察.结果 草酸艾司西酞普兰组的有效率、治愈率分别为66.7%、48.5%,舍曲林组有效率为70.4%,治愈率为48.1%,两组间疗效差异无统计学意义(P>0.05).统计两组的不良反应,草酸艾司西酞普兰组主要表现为食欲下降、恶心、头晕、口干、睡眠障碍,分别占比9.1% (3/33)、6.1% (2/33)、6.1% (2/33)、3.0% (1/33)、6.1% (2/33),与另一组的不良反应率比较接近.两组脱落和失访率分别为12.1%和11.1%,差异很小.结论 对产后抑郁症患者使用草酸艾司西酞普兰治疗的效果、安全性和舍曲林比较相近,对患者安全有效.Objective To investigate the efficacy and safety of escitalopram oxalate and sertraline in the treatment of postpartum depression.Methods 60 postpartum depression patients with CCMD-3 depression were randomly divided into escitalopram oxalate group and sertraline group.The dosage of escitalopram oxalate is 10-20mg/d,sertraline 50-100mg/d5the efficacy was evaluated using the hamilton anxiety scale(HAMA)and hamilton depression scale(HAMD),adverse reactions and safety were assessed by TESS and laboratory tests.The observation period was 6 weeks,Results The effective rate of escitalopram oxalate group was 66.7%,the cure rate was 48.5%,the effective rate of sertraline group was 70,4%,and the cure rate was 48 J%,There was no significant difference between the two groups(P>0.05).The main adverse reactions of escitalopram oxalate were decreased appetite in 3 cases(9.1%),nausea in 2 cases(6.1%),dizziness in 2 cases(6.1%),dry mouth in 1 case(3,0%),and sleep disturbance in 2 cases(6.1%)5there was no difference with sertraline,The drop-out and loss-to-interview rates were 12,1%and 11,1%,respectively,with no statistical difference,Conclusion Escitalopram oxalate is safe and effective in treating postpartum depression patients,the efficacy and adverse reactions are similar to sertraline,
