阴性法CT尿路成像(CTU)在血尿病因检查中的临床鉴别诊断价值

摘　　要：目的探讨阴性法CT尿路成像(CTU)在检出血尿病因方面的价值.方法回顾分析99例肾功能检测正常的血尿患者,所有患者常规行B超后,均使用西门子64排螺旋CT扫描,CT扫描获得原始数据导入后处理工作站snygo.via,采用不同的重建方法进行后处理,主要有曲面重建(CPR)、多平面重建(MPR),所得图像由两名高年资放射科医师(从事放射诊断工作10年以上)进行阅片诊断,所得结论不一致时采取协商的方式得出结论,将CT诊断意见与出院诊断结果采用卡方检验(Chi-square test)χ2进行对比分析,P<0.05作为有统计学差异的标准.结果99例患者中,经阴性法CTU诊断泌尿系肿瘤8例(8.0%),结石75例(75.8%),炎症及结核各1例(1.0%),未明确诊断14例(14.1%).MSCTU在血尿病因诊断的敏感性、特异性、阳性预测值、阴性预测值、准确性分别为93.4%、87.5%、98.0%、91.9%、91.9%,χ2=9.791,P<0.05.结论阴性法CT尿路成像在检出血尿病因方面尤其是泌尿系结石方面有重要的应用价值.Objective To investigate the negative contrast CT urography(CTU)value in the detection of hematuria etiology.Methods Retrospective analysis of 99 cases of normal renal function in patients with hematuria.All patients underwent CT(Siemens 64-slice CT scanner)and ultrasonography.The original data was submitted to the processing workstation syngo.via,and different reconstruction methods were used.There were curved planar reconstruction(CPR),and multi-planar reconstruction(MPR).The image were read by two high qualification radiologists(in diagnostic radiology work for more than 10 years).The conclusion was inconsistent to take consultative manner to solve.Two sets of results using the chi-square test,P<0.05 as statistically significant criteria.Results In 99 patients,the negative method CTU diagnosis of urinary tract cancer in 8 cases(8.0%),stones 75 cases(75.8%),inflammation and tuberculosis in 1 case(1.0%),undetermined 14 cases(14.1%)。The MSCTU sensitivity,specificity,positive predictive value(PPV),negative predictive value(NPV)and diagnostic accuracy was 93.4%、87.5%、98.0%、91.9%、91.9%,respectively,χ2=9.791,P<0.05.Conclusion Negative Method CT urography in the detection of hematuria etiology of urinary calculi in particular has an important application value.
