外阴佩吉特病脑转移一例报告及文献复习

摘　　要：目的分析外阴佩吉特病伴脑转移的临床特点和治疗情况,提高对外阴佩吉特病的认识。方法通过1例外阴佩吉特病脑转移病例,结合文献对治疗方案选择、放疗剂量和脑转移的治疗3个方面进行了分析和综述。结果本例外阴佩吉特病患者采用了手术联合放化疗方案,复发后化疗过程中出现脑部转移灶,行脑部伽马刀治疗5Gy 6次,未出现严重的相关并发症。结论外阴佩吉特病在治疗上应评估患者病情,可行手术的应行根治性切除术联合术后放化疗,出现脑转移行伽马刀也可获得较好疗效。Objective To analyze the clinical characteristics and treatment of vulva Paget's disease with brain metastasis,and to improve the understanding of vulva Paget's disease.Methods Based on a case of vulva Paget's disease with brain metastasis,this paper analyzed and summarized three aspects of treatment plan selection,radiotherapy dose and the treatment of brain metastasis combined with literature.Results In this case,the patient with Paget's disease of the vagina was treated with surgery combined with radiotherapy and chemotherapy.After recurrence,brain metastatic foci occurred in the course of chemotherapy.Gamma knife treatment of 5Gy in the brain was performed for 6 times,and no serious related complications occurred.Conclusions In the treatment of Paget's disease of vulva,the patient's condition should be evaluated.For the feasible operation,radical resection combined with postoperative radiotherapy and chemotherapy should be performed.In case of brain metastasis,gamma knife can also obtain better curative effect.
