摘  要:目的探讨经典型霍奇金淋巴瘤(cHL)^(18)F-氟代脱氧葡萄糖(FDG)正电子发射型计算机断层显像(PET)/CT代谢参数与临床病理特征的相关性。方法回顾性分析2019年1月至2023年1月于空军军医大学第二附属医院初诊的33例cHL患者的临床病理资料和^(18)F-FDG PET/CT影像资料。以标准化摄取值(SUV)=2.5为阈值3D勾画出所有病灶的感兴趣区(ROI),计算出全身病灶的最大SUV(SUV_(max))、肿瘤代谢体积(MTV)和肿瘤糖酵解负荷(TLG)。采用Mann-Whitney U检验分析各组间代谢参数的差异性,采用Spearman相关性分析评估各代谢参数与各临床病理特征的相关性,绘制受试者工作特征(ROC)曲线评估PET/CT各代谢参数在分期中的诊断效能。结果33例cHL患者的SUV_(max)为10.57(8.41,14.71),MTV为146.26(76.12,240.84),TLG为641.01(303.36,1191.91)。PET/CT代谢参数SUV_(max)、MTV、TLG与分期、国际预后评分(IPS)、血清乳酸脱氢酶(LDH)水平呈正相关(均P<0.05),SUV_(max)和MTV与是否结外侵犯呈正相关(均P<0.05),TLG与超敏C反应蛋白(hsCRP)、血沉(ESR)呈正相关(均P<0.05)。SUV_(max)、MTV、TLG与性别、年龄、分子分型、有无B症状、血清β2微球蛋白(β2-MG)、增殖细胞核抗原67(Ki-67)均无明显相关性(均P>0.05)。ROC曲线分析结果显示,在cHL中SUV_(max)、MTV和TLG预测Ⅲ-Ⅳ期病变的界值分别为12.47、195.26和677.64,曲线下面积(AUC)分别为0.884、0.843和0.838,灵敏度、特异度分别为88.9%、83.3%,88.9%、79.2%和88.9%、70.8%。结论^(18)F-FDG PET/CT代谢参数与cHL分期、IPS评分、结外侵犯、LDH、hsCRP、ESR等多种临床病理因素存在相关性,可一定程度反映肿瘤的临床病理特征,为肿瘤特性和侵袭性的判断提供参考。Objective To investigate the correlation between^(18)F-fluorodeoxyglucose(FDG)positron emission computed tomography(PET)/CT metabolic parameters and the clinicopathological features of classical Hodgkin lymphoma(cHL).Methods The clinicopathological data and^(18)F-FDG PET/CT imaging of 33 patients with cHL who were newly diagnosed in the Second Affiliated Hospital of Air Force Medical University from January 2019 to January 2023 were retrospectively analyzed.The standardized uptake value(SUV)2.5 was used as the threshold to draw the region of interest(ROI)of all lesions in 3D.The maximum standardized uptake value(SUV_(max)),metabolic tumor volume(MTV)and tumor glycolytic load(TLG)of the whole lesion were calculated by software.Mann-Whitney U test was used to analyze the differences of metabolic parameters among groups,and Spearman correlation analysis was used to evaluate the correlation between metabolic parameters and clinicopathological features.The receiver operating characteristic(ROC)curve was drawn to evaluate the diagnostic efficacy of PET/CT metabolic parameters in staging.Results A total of 33 patients were enrolled,the SUV_(max),MTV and TLG were 10.57(8.41,14.71),146.26(76.12,240.84)and 641.01(303.36,1191.91),respectively.The PET/CT metabolic parameters SUVmax,MTV and TLG were positively correlated with stage,international prognostic score(IPS)and lactate dehydroge nase(LDH)(all P<0.05).SUV_(max)and MTV were positively correlated with extranodal invasion,and TLG was positively correlated with high-sensitivity C-reactive protein(hsCRP)and erythrocyte sedimentation rate(ESR)(all P<0.05).SUV_(max),MTV and TLG were not significantly correlated with gender,age,subtypes,B symptoms,β2-microglobulin(β2-MG)and proliferating cell nuclear antigen 67(Ki-67)(all P>0.05).ROC curve analysis showed that in cHL,the cut-off values of SUV_(max),MTV and TLG for predicting stageⅢ-Ⅳlesions were 12.47,195.26 and 677.64,respectively,and the AUC were 0.884、0.843 and 0.838,respectively.The sensitivity and specificity wer
