摘 要:目的:对比分析原发性开角型青光眼(POAG)、原发性慢性闭角型青光眼(CPACG)患者和正常人的黄斑区脉络膜厚度及体积差异。方法:病例对照研究。连续性选取2018年10月至2021年10月就诊于石家庄市人民医院眼科的77例(77眼)POAG患者作为POAG组,80例(80眼)CPACG患者作为CPACG组,年龄、眼压及眼轴匹配的健康者79例(79眼)作为对照组。采用增强深度成像光学相干断层扫描(EDI-OCT)测量所有受检者黄斑9个区脉络膜厚度和体积。通过单因素方差分析对3组受检者的黄斑区脉络膜厚度和体积进行比较,采用Pearson相关分析对青光眼患者黄斑区脉络膜厚度与视野平均缺损(MD)的相关性进行分析。结果:POAG组、CPACG组及正常对照组黄斑区脉络膜厚度平均值、中央区、鼻内环、上内环、颞内环、下内环、鼻外环、上外环、颞外环、下外环差异均有统计学意义(F=9.87,P<0.001;F=7.23,P<0.001;F=9.90,P<0.001;F=8.16,P<0.001;F=6.56,P=0.002;F=10.69,P<0.001;F=7.89,P<0.001;F=8.51,P<0.001;F=6.61,P=0.002;F=9.93,P<0.001);3组黄斑区脉络膜体积平均值、中央区、鼻内环、上内环、颞内环、下内环、鼻外环、上外环、颞外环、下外环体积差异有统计学意义(F=9.80,P<0.001;F=7.03,P<0.001;F=11.39,P<0.001;F=7.85,P<0.001;F=5.59,P=0.004;F=10.01,P<0.001;F=6.55,P=0.002;F=8.44,P<0.001;F=6.43,P=0.002;F=9.94,P<0.001)。POAG组较对照组黄斑9个区及平均黄斑区脉络膜厚度和体积差异均无统计学意义(均P>0.05);CPACG组较POAG组和对照组黄斑9个区及平均黄斑区脉络膜厚度均变厚和体积均变大,差异均有统计学意义(均P<0.05)。POAG组和CPACG组黄斑9个区脉络膜厚度与视野MD均无相关性(P>0.05)。结论:CPACG患者较POAG患者和正常人黄斑区脉络膜厚度均变厚、体积均变大,脉络膜在CPACG患者的发病机制中可能起重要作用。Objective:To investigate differences in macular choroid thickness and volume between primary openangle glaucoma(POAG),primary chronic angle-closure glaucoma(CPACG),and normal controls.Methods:This was a case-controlled study.Patients admitted to Shijiazhuang People's Hospital from October 2018 to October 2021 were selected in this study.A total of 77 patients(77 eyes)with POAG were included as the POAG group,80 patients(80 eyes)with CPACG were included as the CPACG group,and 79 normal subjects(79 eyes)with matched age,intraocular pressure,and axial length were selected as the control group.Thickness and volume of choroid in 9 macular regions of all subjects measured by Enhanced depth imaging optical coherence tomography(EDI-OCT)was analyzed by one-way variance measurement.And Pearson correlation analysis was used to analyze the correlation between macular choroid thickness and visual field mean defect(MD)in glaucoma patients.Results:The choroidal thicknesses of the average macula,central subfield macula,nasal inner macula,superior inner macula,temporal inner macula,inferior inner macula,nasal outer macula,superior outer macula,temporal outer macula and inferior outer macula regions between the POAG group,the CPACG group,and the normal control group were statistically significant(F=9.87,P<0.001;F=7.23,P<0.001;F=9.90,P<0.001;F=8.16,P<0.001;F=6.56,P=0.002;F=10.69,P<0.001;F=7.89,P<0.001;F=8.51,P<0.001;F=6.61,P=0.002;F=9.93,P<0.001).The choroidal volume of the average macula,central subfield macula,nasal inner macula,superior inner macula,temporal inner macula,inferior inner macula,nasal outer macula,superior outer macula,temporal outer macula and inferior outer macula regions between three groups were statistically significant(F=9.80,P<0.001;F=7.03,P<0.001;F=11.39,P<0.001;F=7.85,P<0.001;F=5.59,P=0.004;F=10.01,P<0.001;F=6.55,P=0.002;F=8.44,P<0.001;F=6.43,P=0.002;F=9.94,P<0.001).There were no significant differences in the thickness and volume of choroid in 9 macular regions and average thickness and volume of choroid
