大剂量化疗治疗小儿急性淋巴细胞白血病的效果观察

摘　　要：目的探讨大剂量化疗治疗小儿急性淋巴细胞白血病(ALL)的临床疗效。方法抽取2020年1月至2022年1月河南省儿童医院收治的ALL患儿104例,根据双色球法分为对照组与观察组,每组52例。两组化疗方案均为6-硫基嘌呤(6-MP)、甲氨蝶呤(MTX)联合三联鞘注,对照组采用常规剂量MTX(3 g/m^(2)),观察组采用大剂量MTX(5 g/m^(2))。比较两组患者的治疗效果、治疗依从性及不良反应发生情况。结果观察组总有效率(88.46%,46/52)高于对照组(71.15%,37/52),P<0.05;两组治疗依从性比较,差异未见统计学意义(P>0.05);两组不良反应发生情况比较,差异未见统计学意义(P>0.05)。结论6-MP、HD-MTX联合三联鞘注方案治疗小儿ALL,采用大剂量MTX治疗不会影响治疗依从性,也不会显著增加不良反应,但可以明显提高治疗效果。Objective To investigate the clinical efficacy of high-dose chemotherapy in the treatment of acute lymphoblastic leukemia(ALL)in children.Methods A total of 104 children with ALL admitted to Henan Children’s Hospital from January 2020 to January 2022 were selected.According to the dichromatic sphere method,they were divided into control group and observation group,with 52 cases in each group.The chemotherapy protocol of the two group were 6-thiopurine(6-MP)and methotrexate(MTX)combined with triple intrathecal therapy.The control group received a conventional dose of MTX(3 g/m^(2)),while the observation group received a large dose of MTX(5 g/m^(2)).The therapeutic effect,treatment compliance,and incidence of adverse reactions were compared between the two groups.Results The total effective rate in the observation group(88.46%,46/52)was higher than that in the control group(71.15%,37/52),P<0.05.There was no significant difference in treatment compliance between the two groups(P>0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions The combination of 6-MP,HD-MTX and triple intrathecal therapy in the treatment of pediatric ALL does not affect treatment compliance or significantly increase adverse reactions,but can significantly improve the treatment effect.
