高频超声联合剪切波弹性成像对乳腺癌的诊断价值

摘  要:目的探讨高频超声联合剪切波弹性成像(SWE)在乳腺癌诊断中的应用价值。方法抽取2019年10月至2021年9月郑州大学第二附属医院收治的86例乳腺占位性病变患者为研究对象。患者均行高频超声联合SWE检查,获取病灶处超声参数[最大血流值(Vmax)、搏动指数(PI)、阻力指数(RI)]及病灶处SWE参数[杨氏模量最大值(Emax)、杨氏模量平均值(Emean)]。以乳腺肿块穿刺活检结果为金标准,分析高频超声联合SWE对乳腺癌的评估价值。结果乳腺肿块穿刺活检结果显示,86例乳腺占位性病变患者中乳腺癌58例(67.44%),乳腺纤维腺瘤28例(32.56%)。以乳腺肿块穿刺活检结果为金标准,高频超声检查结果显示,乳腺癌组Vmax、PI均高于非乳腺癌组(P均<0.05);SWE检查结果显示,乳腺癌组Emax、Emean均大于非乳腺癌组(P均<0.05)。绘制受试者工作特征曲线,结果显示,Vmax、PI、Emax、Emean及联合的曲线下面积均≥0.7,当各指标取最佳阈值时,可获得最佳诊断价值,其中联合诊断价值高于单一指标诊断。结论高频超声联合SWE检查可检测乳腺占位性病变处血流动力学改变,并定量评价病灶硬度,为乳腺癌患者术前诊断提供科学的影像学支持。Objective To investigate the value of high frequency ultrasound combined with shear wave elastography(SWE)in the diagnosis of breast cancer.Methods Eighty-six patients with space-occupying lesions in the breast treated in the Second Affiliated Hospital of Zhengzhou University from October 2019 to September 2021 were selected as research objects.All patients underwent high frequency ultrasound combined with SWE to obtain ultrasound parameters at the lesions,including maximum blood flow value(Vmax),pulsatility index(PI)and resistance index(RI),and SWE parameters,including the maximum value of Young’s modulus(Emax)and the average value of Young’s modulus(Emean).The evaluation value of high-frequency ultrasound combined with SWE for breast cancer was analyzed with the results of breast mass biopsy as the gold standard.Results Among the 86 patients with space-occupying lesions in the breast,58 cases(67.44%)had breast cancer,and 28 cases(32.56%)had fibroadenoma.Taking the results of breast mass biopsy as the gold standard,the results of high frequency ultrasound showed that Vmax and PI in the breast cancer group were higher than those in the non breast cancer group(all P<0.05);results of SWE showed that Emax and Emean in the breast cancer group were significantly higher than those in the non breast cancer group(all P<0.05).Results of receiver operating characteristic curve showed that the area under the curve of Vmax,PI,Emax,Emean and their combination were≥0.7.When the best threshold value was taken for each index,the best diagnostic value could be obtained,and the diagnostic value of the combination was higher than that of Vmax,PI,Emax,Emean alone.Conclusions High frequency ultrasound combined with SWE can detect the hemodynamic changes of space-occupying lesions in the breast and quantitatively evaluate the hardness of lesions,providing scientific imaging support for preoperative diagnosis of breast cancer patients.
