摘  要:晚期食管癌以放化疗综合治疗为主,而食管癌干细胞(ECSC)的存在是肿瘤对化疗和放疗抗拒以及治疗后进展失败的重要原因。这部分细胞的高致瘤性、高侵袭性和多重耐药等特性,最终导致治疗失败。研究证实,ECSC的表面标志物可作为预测食管癌患者放化疗疗效和预后的指标。肿瘤相关的多信号通路途径以及肿瘤微环境也参与肿瘤干细胞的发生、侵袭、转移以及对放化疗抗拒等环节。因此,深入研究ECSC放化疗抗拒的机制将有助于实现食管癌治疗新策略。Radiotherapy and chemotherapy are the main treatment for advanced esophageal cancer,and esophageal cancer stem cells(ECSC)are mainly responsible for chemoradiotherapy resistance and treatment failure.These cancer stem cells(CSC)harbor unique properties including high tumorigenicity,proliferation,migration,and resistance to conventional therapy,leading to treatment failure.Studies have confirmed that the surface markers of ECSC can predict the efficacy and prognosis of radiotherapy and chemotherapy in patients with esophageal cancer.In addition,multiple tumor signal pathways and tumor microenvironment are also involved in the occurrence,invasion,metastasis,and chemoradiotherapy resistance of CSC.Therefore,an in-depth study of the mechanism of chemoradiotherapy resistance in ECSC will help to achieve new strategies for the treatment of esophageal cancer.
