摘 要:目的探讨分层次阶段式带教对神经外科实习教学的效果。方法选取2019年7月至2020年3月在神经外科实习的35名实习生为对照组,采取传统临床带教;选取2020年6月至2021年2月的34名实习生为研究组,采取分层次阶段式带教。带教后比较出科考核成绩,并比较实习前后迷你临床演练测评量表(mini-clinical evaluation exercise,Mini-CEX)评分及实习生对带教方法的认可度。采用SPSS 24.0进行t检验和卡方检验。结果研究组实习后的神经外科理论知识成绩[(90.15±4.47)分]、操作技能成绩[(91.12±3.68)分]、临床病例分析能力考核成绩[(87.32±5.41)分]均高于对照组[(85.26±5.19)分、(86.14±5.25)分、(80.94±4.26)分],差异有统计学意义(P<0.001);两组实习后Mini-CEX量表评分均高于实习前(P<0.001),研究组实习后均高于对照组(P<0.001);研究组实习生对带教方法能提高学习兴趣与积极性、提高牢固掌握知识能力、提高临床问题分析与处理能力、提高理论知识与实践结合能力、提高临床思维能力方面的认可度均高于对照组(P<0.05)。结论在神经外科临床实习教学中采取分层次阶段式带教,可提高实习生的综合考核能力与专业素养,并可提高其对带教方法的认可度。Objective To explore the teaching effect of hierarchical and phased teaching method on the teaching of neurosurgery interns.Methods Thirty-five interns of the Department of Neurosurgery of The First People's Hospital of Liangshan Yi Autonomous Prefecture from July 2019 to March 2020 were selected as the control group,and received the traditional clinical teaching method.Another 34 interns from June 2020 to February 2021 were selected as the research group,and adopted the hierarchical and phased teaching method.The graduation examination results were compared,and the scores of mini-clinical evaluation exercise(Mini-CEX)scale before and after practice and the recognition of teaching methods by interns were compared.SPSS 24.0 was used for t test and chi-square test.Results The results of theoretical knowledge of neurosurgery[(90.15±4.47)points],operation skills[(91.12±3.68)points]and clinical case analysis ability[(87.32±5.41)points]in the study group were higher than those in the control group[(85.26±5.19)points,(86.14±5.25)points and(80.94±4.26)points],and the differences were statistically significant(P<0.001).The scores of Mini-CEX scale in the two groups after internship were both higher than those before internship(P<0.001),which in the study group after internship were higher than those in the control group(P<0.001).The recognition of the teaching method in improving the learning interest and enthusiasm,strengthening the ability to master knowledge,improving the ability to analyze and deal with clinical problems,improving the ability to combine theoretical knowledge with practice and improving the clinical thinking ability of interns in the study group were higher than those in the control group(P<0.05).Conclusion In the clinical teaching of neurosurgery interns,the application of hierarchical and phased teaching method can significantly improve the comprehensive assessment ability and professional quality of interns,and can also improve their recognition of this teaching method.
