加强癫痫诊疗规范化对改善患者生活质量的分析研究

摘　　要：目的探讨加强规范化诊疗对成年癫痫患者生活质量的影响。方法80例临床确诊的成年癫痫患者,病程至少1年以上,发作频率大于4次/月,前期诊疗不规范。所有患者采取严格诊疗程序,采用成年癫痫生活质量量表-31(QOL-31中文版)评估抗癫痫药治疗3个月对患者生活质量各个方面的影响。结果发作长期未控制的癫痫患者生活质量各方面均明显下降,其中以发作担忧和药物影响评分最低,同时性别、文化程度、职业、经济状况和居住地对患者总的生活质量及生活质量的各方面有一定的影响;规范化治疗后患者总生活质量及各方面均明显改善(P<0.05),同时消除或明显改善了上述因素对生活质量的影响。结论加强癫痫诊疗规范化是控制癫痫发作、提高患者生活质量的关键。Objective To study the effects on improving quality of life(QOL) in adult patients with epilepsy by enhancing the standardization of diagnosis and treatment.Methods Eighty adult patients with epilepsy under nonstandard diagnosis and treatment,whose disease course was at least one year and seizure frequency more than four times a month were enrolled standardization with strict procedure of diagnosis and treatment was then adopted.After three months' standard AEDs treatment,the QOL for these patients were evaluated by quality of life in epilepsy inventory-31(QOLIE-31,Chinese version).Results All aspects of QOL for patients with uncontrolled seizures had been decreased,among which the values of worry about seizure and effects of medication were lowest.At the same time gender,education background,occupation,economic status and living condition also affected the overall QOL and various scales of QOL.After standard treatment overall QOL and all scales were significantly improved,meanwhile the influence of the above factors on QOL were largely eliminated or obviously diminished.Conclusion Enhancing the standardization of diagnosis and treatment was the key to controlling seizure and improving QOL in adult patients with epilepsy.
